Position summary: This is a grant funded position. Diabetes Program Nurse Practitioner is primarily responsible for providing medical management, care and education for persons diagnosed with diabetes and related chronic conditions, as well as prevention and wellness. This position will also assist with ongoing program planning, development and implementation of diabetes-related chronic disease prevention and treatment projects Conduct regular trips to village clinics to provide patient care. Collaborate with YKDRH and Sub-regional clinic providers to ensure patients are receiving care according to American Diabetes Association’s Standards of Care. Provide technical assistance and training to medical staff on Diabetes issues and current standards of care. Assist with diabetes-related health promotion projects, visit tribal councils, and function as a Diabetes Team Member. Believe in and promote Napartet and all corporate endeavors. Position Qualifications: Master’s Degree in the Science of Nursing; preferred Master’s Degree in Public Health (MPH). Two (2) years experience in family practice, chronic disease management or related clinical setting. Preferred experience working in public health programming.
