Our Caregivers provide, personal care, transportation, and support services to clients in a Comfort Keepers established Personal Care Program Location: Chandler, AZ Essential Functions: Provides general attention to clients non-medical needs in accordance with an established Plan of Care. Provides for a clean, safe, and healthy environment for clients by completing light housekeeping tasks including laundering of client’s garments and linens. May prepare and serve meals as directed. Ensures that dishes are washed and kitchen clean after each meal. Assists client in completing necessary phone calls, letter writing, etc. Accompanies client on walks, community trips, doctor’s office, bank, beauty salons, etc. Reminds client to take self-administered medications. Observes and reports any changes in the client’s mental, physical, or emotional condition or home situation to immediate supervisor in a timely manner. Establishes and maintains effective communication and a professional relationship with clients, family members, and co-workers. Participates in in-service and continuing education programs, staff meetings, and client conferences as requested by immediate supervisor. Completed required documentation of services delivered and submits to office in a timely manner according to policy. Uses equipment and supplies safely and properly. Maintains confidentiality regarding client information. Provides companionship for the client including, but not limited to: talking and listening, reading aloud, providing social and emotional support. Promotes the client’s mental alertness through involvement in activities of interest. Provides emotional support and promotes a sense of well-being. Other reasonable related duties as assigned. Qualifications: Must demonstrate satisfactory completions of any stated mandated training. Provides all investigative documentation to the Division QMU Clinical Investigator to assist them in the investigative and severity determination process of the Quality of Care (QOC) Investigation. Follows up with vendors regarding Corrective Action Plan (CAP) Remediation follow up. Reports all Health and Safety and QOCs to DDD QMU. • Provides the vendor agencies, service providers and Division staff with ongoing training, technical assistance and continued on-site monitoring to reduce the likelihood of the incident reoccurring and promoting vendor/provider regulatory compliance. • Completes incident entry of the IR into IMS. Notifies key clinical triage personnel of incidents & notifies other depts who may need to conduct an immediate follow up for the incident. Cross checks incident with client application database & District Employee Directory to verify information. • Answers incident reporting phone line, ensures calls are entered into IMS, follows up with after hour emergency line designated contact to ensure all incidents are being entered. Conducts follow up visits w/vendors for written reports on incidents that were verbal notifications. • Drive to and from licensed settings to complete clinical health and safety visits. CVICU Nurse

Responsibilities Chandler Regional Medical Center’s Intensive Care Units provide patient care to Level One Trauma, Cardiac Surgery, ECMO, Tandem, Neurology, Neurosurgery and Medical patients. Our ICU is an 80 bed ICU that consists of 36 Medical-Surgical Beds (12 of which are CVICU), 32 Surgical-Trauma-Neuro Beds, and 12 Cardiac Care Beds which all house state of the art equipment. Chandler is ACS Level One Certified Trauma Center, Accredited Stroke Center, and is a Center of Excellence for Coronary Artery Bypass Patients and Mitral Valve replacements. The ratio for our ICU patients is 1:1 or 1:2 and is staffed 24x7 by certified Critical Care Intensivists. The ICU has both a house wide as well as a unit based Shared Governance, that is nurse driven to allow staff the autonomy and buy in into how the unit is run. We utilize self-scheduling. The four ICUs are led by a shared leadership team, which provides many opportunities for growth, as well as the ability to cross train within the various ICUs (Trauma, Neuro, Surgical, Medical, and CVICU). MICU As a Medical ICU Nurse, you will provide direct nursing care to patients with sepsis, DKA, post cardiac arrest, respiratory failure and various other medical conditions for this 36-bed unit to medical ICU.
