N2S Heart Failure Unit - 32 bed Telemetry/Heart Failure Accredited Unit providing comprehensive care to patients with complex cardiac conditions including heart failure, chest pain, dysrhythmias, syncope, pre and post cardiac procedures, and respiratory conditions. The team delivers compassionate, comprehensive care to hemodynamically stable patients requiring close monitoring of certain cardiac drips. The staff is comprised of a team of clinical registered nurses and patient care assistants who collaborate with providers, physical and occupational therapists, dietitians, clinical nurse specialists, respiratory therapists, and care coordinators. N2W Heart Failure & Cardiac Unit - 31 bed unit specializing in Heart Failure, peri-interventional cardiology care, and medical telemetry. The energetic and dynamic nursing team provides high quality, patient centered care for those who do not require critical care nursing, but who have complex cardiovascular conditions. These patients commonly have the diagnoses of CHF, ACS, AMI, dysrhythmia, or syncope in addition to other concomitant medical diagnoses. C3 Medical Unit - 27 bed unit. Common diagnoses include sepsis, cellulitis, gastrointestinal bleed, pneumonia, diabetic complications, and medical management pre- and post- liver transplant. C3’s care team includes close collaboration with physicians, social workers, case management, and pharmacists to provide comprehensive care. Job Description: Summary The Licensed Practical Nurse (LPN) performs nursing functions within the scope of practice and provides and/or coordinates care and services to meet the residents’ physical, psychosocial, and functional care needs. Job Responsibilities 1. Practices nursing care within the scope of licensure and in accordance with the state Nurse Practice Act; Keeps a current active license; Follows state and federal licensure rules and regulations for the level of care in which they practice and in accordance with EverTrue clinical and administrative policies and procedures 2. Works collaboratively with the interdisciplinary team in defining individualized plans of care for each resident; Works closely with nursing leadership to ensure regulatory compliance; Promotes individual and team learning and provides teaching and role modeling of professional nursing practices to staff for quality clinical care 3. Passes medication and/or oversees the medication ordering and administration process; Administers medications and treatments to residents; monitors administration of I.V. fluids, supplemental nourishments, and medications when ordered 4. May initiate IV therapy if the LPN is IV-certified 5. Maintains a working knowledge and understanding of the observation/assessment and documentation process for the level of licensure in which they work, including care and service plans and the MDS as applicable 6. whenever leaving unit Assists residents to attain or maintain their highest practicable level (physically, mentally, and psychologically) Utilizes resident specific information as conversation topics Assists residents to receive care in a timeframe and manner according to their preferences Reports changes in resident status, resident response to care, equipment and supply needs, repairs and maintenance needs, safety hazards, unusually occurrences or observations, and completion of assignments to licensed staff Documents resident care accurately and in a timely manner Respects residents’ rights Contributes to the residents care plan by providing resident-specific physical, psychosocial, cultural, and spiritual information derived through care giving activities Assists with resident admissions, transfers, and discharges Prepares rooms for new admissions Provides supportive care to residents as end of life Demonstrates knowledge of working with persons with cognitive loss MAINTAINS CONFIDENTIALITY Complies with all aspects HIPAA Conforms with organizational policies and procedures with regard to maintaining confidentiality of resident and organizational information to: bathing (bed bath, tub, shower), feeding, toileting, perineal care, grooming (hair, nails, foot care), shampooing residents’ hair, oral /denture care, measuring intake and output, transfers from bed, chair, and wheelchair, assisting residents in use of assistive devices, positioning residents in chairs, beds, wheelchairs, gericharis, turning and changing resident position, lifting residents with mechanical lifts, performing both passive and active range of motion exercises, making occupied and unoccupied beds, obtaining resident weights, taking vital signs, providing post-mortem care, practicing universal precautions and special isolation techniques, monitoring devices as per plan of care Transporting and escorting residents as needed ENSURES RESIDENT SAFETY Demonstrates knowledge of all organizational emergency codes and participates effectively Demonstrates knowledge and skill in resident transfers, oral feeding, and provision of personal hygiene Protects residents from compilations and injuries Responds to the concerns, potential or actual injury, and/or distress of all residents, visitors, and staff Protects residents from abuse and neglect and reports suspected and/or actual abuse to supervisor MAINTAINS A SAFE & CLEAN WORKING ENVIROMENT Complies with Infection Control policies, procedures, and regulations Complies with organizational / Infection Control standards on storage and disposal of toxic and non-toxic chemicals and other wastes Coordinates with health care support personnel as necessary Keep equipment, supplies, and residents furniture clean
