Serenity Hospice is looking to hire 'Registered Nurse (RN) Case Managers to join our team of professionals. RN Case Manager will provide service in the York County PA area. The RN Hospice Case Manager will be responsible for assisting the Team with Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to: Completing an initial assessment of patient and family to determine hospice care needs Regularly re-evaluates the patients' nursing needs Use assessment data to determine care plan needs Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide are that is continuous and well-coordinated Counsels the patient and family in meeting end of life needs Initiates appropriate preventative nursing procedures Communicates with the physician regarding patient's needs and reports any changes in the patient's condition Supervises the Hospice Aides and LPN's The proper candidate must have a current Pennsylvania RN License in good standing, must be familiar with and willing to travel the area and have hospice experience. Job Description: Serenity Hospice is seeking a compassionate and skilled Certified Registered Nurse Practitioner (CRNP) to join our exceptional team. As a CRNP, you will play a critical role in ensuring the highest quality of end-of-life care for our patients and their families. Your primary responsibility will be to conduct face-to-face visits with hospice patients to evaluate their eligibility status in accordance with CMS Hospice Eligibility Guidelines. Key Responsibilities: Conduct face-to-face visits with hospice patients to assess eligibility based on CMS guidelines. Collaborate and communicate with the Hospice Medical Director and/or the patient’s attending physician to develop and adjust patient care plans. Provide clinical expertise to the interdisciplinary hospice team to support optimal patient care. Authorize medication orders as applicable per state laws and patient needs. Ensure documentation is accurate and timely to meet regulatory and organizational standards. Offer guidance and recommendations to the hospice care team on symptom management and patient care strategies. Participate in team discussions, IDG meetings, and care planning sessions as needed. Work primarily from an office setting, with occasional travel to patient homes or care facilities within a reasonable distance. Qualifications: Current and valid CRNP license in Pensylvania. Experience in hospice or palliative required. Strong understanding of CMS Hospice Eligibility Guidelines. Ability to work effectively in a team-oriented, interdisciplinary environment. Excellent communication, documentation, and organizational skills. Willingness to travel as necessary for patient visits (within reason).
