BRIEF JOB DESCRIPTION: The Registered Nurse (RN)/Case Manager plans, organizes, and directs home care services emphasizing health education/experience. The Registered Nurse (RN)/Case Manager is responsible for traveling to a patient’s home to administer the services dictated by their physician while helping patients maintain their independence and quality of life. This role provides a lot of FLEXIBILITY for our field staff! Full, part-time, and PRN positions availabe in the Lake County area. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES FOR REGISTERED NURSE/CASE MANAGER (RN): Patient Care: 1. The Registered Nurse (RN) assess and chart observations of the patient's condition and needs at each home visit. 2. Complete evaluation tasks, including medication reviews and vital signs. 3. Initiate the Plan of Care and makes necessary revisions as patient status and needs change. 4. Provide education to patients and family members on proper home health procedures and strategies. 5. Develop a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive and rehabilitative nursing actions. 6. Administer medications and treatments as prescribed by the physician. 7. Provides health care instructions to the patient as appropriate per assessment and plan of care. 8. Identifies discharge planning needs as part of the care plan. Prepares patient and family for discharge. 9. RN acts as Case Manager when assigned by Director of Nursing and assumes responsibility to coordinate patient care for assigned caseload. 10. Coordinate with physicians, physical therapists, occupational therapists, MSWs and other individuals in the patient care plan. Perform a supervisory visit with CNA (when part of the care plan) once every two (2) weeks. Communication: 1. Prepares clinical notes and updates the primary physician when necessary. 2. Communicate with the physician and office regarding the patient's needs and report any changes in the patient's condition; obtain/receives physician's orders as required. 3. Communicate with community health-related persons to coordinate the care plan. 4. Registered Nurse (RN) reports with home office on any updates, concerns and issues regarding the paient. POSITION QUALIFICATIONS: 1. Home healthcare experience preferred. 2. Graduate of an accredited school of nursing. 3. Current licensure in state of Indiana and current CPR certification. 4. Management experience not required. Responsible for supervising home health aides. 5. Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, nursing skills per competency checklist. 6. Prolonged or considerable walking or standing. These types of cases are based on patient diagnoses and include a variety of issues ranging from severe trauma, acute illnesses, to chronic conditions and elective surgeries. Specialties performed at Arnett include general, gynecology, urology, orthopedics, neurology, vascular, cardiac, ENT, and ophthalmology. The patient population in an OR can vary widely depending on the hospital's specialty and services. We have 13 total ORs between the Main OR and the ASC OR. We have 2 dedicated rooms for Robotic Surgery. We have a dedicated cardiac room. We recently added a hybrid OR. A hybrid operating room is a highly advanced surgical space that integrates the capabilities of a traditional OR with cutting-edge imaging technology, such as CT, MRI, or fluoroscopy. This allows surgeons to perform both open and minimally invasive procedures in the same room, often without needing to move the patient. For nurses, especially in the OR, this means being part of a dynamic, fast-paced setting that emphasizes teamwork, adaptability, and advanced clinical skills. Nursing skills gained: Nursing skills gained while working in the OR include learning about aseptic technique, as well as maintaining a sterile field, proper gowning/gloving, and preventing contamination during procedures. Nurses in the OR do patient assessments including preoperative checks, intraoperative hemodynamic monitoring, patient advocacy, and postoperative hand-offs. OR nurses gain surgical instrumentation knowledge such as how to identify and handle instruments, anticipate surgeon needs during procedures and ensure proper sterilization and set up. The RN supports anesthesia by assisting with intubation/emergence, monitoring patient responses to anesthesia, managing airways and helping with lines and equipment. RNs perform proper patient positioning using padding and support to prevent injury and give the surgeon the optimal view of the surgical site. They are also responsible for documentation of the surgical procedure including accurate intraoperative records, timeouts, safety checklists, and specimen labeling and tracking. OR circulators are part of a team that coordinate care with surgeons, anesthesiologists, and surgical technologists. They must be patient advocates and stay up to date on their emergency preparedness skills including recognition of complications (hemorrhage, anaphylaxis). They often assist in resuscitation or rapid interventions during codes. They must maintain ACLS and PALS certifications which is also offered at IU Arnett. Unit Successes/Designations/National recognition: We are a level 3 trauma hospital. A Trauma 3 Hospital, or Level III trauma center, provides essential care for injured patients, offering prompt assessment, resuscitation, and stabilization, but may not have the full range of specialized services found in higher-level trauma centers. DSD RNs will troubleshoot, and make sound decisions for our patients related to their dialysis treatment and will occasionally partner alongside the SNF staff to coordinate care. Patient education. RNs by training, are Educators. As a DSD Services RN, you will be provided the opportunity to conduct impactful and meaningful education to patients and their families in the SNF setting. Direct Patient Care: The RN’s primary responsibility is to independently demonstrate best practice techniques and deliver optimal dialysis care. The role includes supervising Dialysis Technician who are tasked with delivering tandem care to meet the capacity and efficiency goals of the program. Complexity - DSD Dialysis care is a complex but fulfilling service line.
