Responsibilities: Manage the initial assessment for potential PACE participants to include home visits to support environmental and physical assessments. Ensure coordination and completion of reassessments every 6 months with 1:1 at PACE Multipurpose Center and/or at participant’s home for assigned case load. Act as a liaison between the participants and the interdisciplinary team members. Provide individual health education on the disease process to include verbal and written hand-outs for their reference. Document any activities encountered between the participant, families, caregivers, contracted providers, hospitals, SNF, and other facilities working together with PACE for participant’s well-being in accordance with PACE policies, procedures, and all regulatory requirements and laws. Ensure implementation and compliance of participant care as per provider’s order. Respond to medical emergencies and take the lead in management of the situation as indicated. Screen and triage participants as needed. Document all nursing care on the day service is rendered while adhering to established guidelines related to medical care provision. Monitor, administer, and record ordered medications. Participate as an active member of the interdisciplinary team to assess the nursing need of the participants. Maintain acceptable standards of nursing care based on regulatory requirements. Perform other duties as assigned. Requirements: Current California state licensure as a Registered Nurse. Current American Heart Association healthcare provider CPR (BLS), or Advanced Cardiac Life Support, Advanced Trauma Life Support, or Advance Resuscitation Training. Traveling between sites and other locations is required as an essential function of the job. Must have a car, a valid California driver’s license, and proof of minimum levels of car insurance as required under California law, although limits of $100,000 are recommended. An acceptable driving record is also required. California law requires all drivers to obtain a valid California driver’s license within ten days of establishing residency. Mileage and other reimbursement governed by policy. 1-3 years of experience working with the older adult population required. 1-3 years of experience in managed care setting to include utilization or case management in an ambulatory or inpatient setting preferred.
