In this role the Certified Nursing Assistant will: Reports directly to the nurse manager, may also take direction from licensed nurses on duty or other supervisory staff. Provides direct care to residents on a daily basis. Position also assists with activities, light housekeeping, laundry, and dining services. Position is service in nature, in performance of duties the resident care assistant is entrusted to assist residents with basic personal care according to procedures and daily assignment. This includes but is not limited to bathing, dressing, dental hygiene, toilet activities, grooming, feeding, transportation/transfer, etc. Maintain resident rooms in a clean and orderly manner by providing light housekeeping, changing linen, making beds, putting away clothing and personal effects, tidying bedside table, shelves, closets, and cleaning bathrooms, etc. Assist with resident laundry assuring clothes are marked, clean and mended, ironed, and put away. Notify supervisor of clothing needs as they occur. Prepare residents for meals, assist with serving meals and feeding of residents. May be cross trained in Assisted Living. The ideal candidate will have: High School diploma or the equivalent. Completion and passing the test for an approved Nursing Assistant program in the State. Listed on the State’s Health Nursing Assistant Registry. #NursingSupport Compensation Pay Range: $19.13 - $23.33 per hour Reasonable Pay Estimate A reasonable estimate of the pay range for this position is $19.13 - $23.33 per hour. Incorporates patient care goals established in the Plan of Care, into nursing visits, as evidenced by documentation on skilled nursing notes. Communicates significant findings, problems and changes in condition or environment to the RN Case Manager, the physician, clinical supervisor, and/or other personnel involved with patient care. Reports unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician, as appropriate. Implements the plan for patient safety, using the patient, family, and community resources. Assists the patient in learning appropriate self-care techniques while delivering quality patient care. Suggests revisions to the plan of care in response to identified patient care issues and notifies the RN Case Manager. Provides those services required substantial and specialized nursing skills under the direction of a Registered Nurse or physician. Collaborate with the RN Case Manager during normal business hours for advice, assistance, and instructions. Documents medication regimen and updates medication profile according to established procedures. Treats patients and caregivers in the highest and most effective manner. Implement all available actions to prevent avoidable hospitalizations and ER visits. Provides positive, supportive communication to physicians, patients, families, visitors, and agency personnel. What is Required? Graduate of an accredited Certificate, Diploma, or Associate School of Nursing Current State License as a Licensed Vocational / Practical Nurse 1 year of experience in a clinical care setting Experience in a hospice or home health environment is preferred 50% travel required Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs. Valid driver’s license and insurance and reliable transportation to perform job tasks The base salary for this position is $35 to $45 per visit, based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity. You will report to the Clinical Team Manager.
