Job Requirements The Home Training Registered Nurse evaluates, trains, and provides support to the dialysis facility’s home patients, care partners, and families. This position exercises knowledge of patient rights and ensures an atmosphere that allows for the privacy, dignity, and well-being of all patients in a safe, secure environment. The position must demonstrate familiarity with dialysis procedures and the knowledge, skills, and judgment necessary to assume the responsibility of caring for the dialysis patient. Work Experience Registered Professional Nurse with current state licensure. Twelve (12) months of experience in nursing. Meets all applicable state/licensing requirements. Additional three (3) months of experience in the specific modality unless otherwise specified in specific state regulations. Documented training and experience in peritoneal dialysis and/or home hemodialysis and maintenance of the dialysis access. Basic training certificates if less than one (1) year in this modality. Previous supervisory and teaching experience preferred. Basic Life Support (BLS) certification. Current CPR Certification Demonstrated ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Assist in developing a written plan of care for each customer that identifies the problems/needs of the customer and be able to evaluate the delivery of care. Attends and actively participate in customer care plan conference. Consult with other nurses, management, and other related health professionals to assist in assessing, planning, and delivering and evaluating patient care. Plan, conduct and participate in In-service Education Programs, workshops, seminars, conferences, and courses. Make written and oral reports including recommendations concerning the wound care program. Ensure that the wound assessment reflects that the care plan is being followed. Participate in the development, maintenance, and implementation of the facility’s Quality Assurance program for the Wound Care Program. Coordinate facility wound audits with the nursing staff. Make daily rounds to assure that nursing personnel are performing required duties related to the wound care program, and to assure that wound care protocols and procedures are being followed. Contact families regarding significant decline in resident's status or transfer to hospital documenting contact in the EMR. Make rounds and observe individual residents who are experiencing episodes of acute illness, deterioration in health status, recent injury, recovering from surgery, etc., so as to be acquainted with the resident's status, both physically and emotionally and to ascertain that the staff is rendering proper care. At all times abide by policies of the facility and ascertain that employees under his/her supervision do the same. Supervise serving of prescribed diets and fluid intake (it is acceptable to document only deviations from normal) and report persistent unresolved problems to the DON. Ensure that all medications and treatments are charted by the person administering the medication or completing the treatment on his/her assigned shift (EMR system). Prepare incident/accident reports, events and observations using the EMR system. Ensure that a copy of the medical records or an abstract of them are forwarded with the transfer of residents to a hospital or other health care facility.
