This position will oversee patient case load while coordinating all patient services with members of interdisciplinary team. This position will perform initial patient evaluation assessment to identify nursing diagnosis and treatment of care in order to meet the needs of the patient. Revise care plan as needed Provide direct patient care and facilitate focused communication with client and family, establishment of the nursing care plan, interventions, and evaluate outcome Coordinate all services and collaborate with the interdisciplinary team as needed Communicate with physicians to confirm, correlate, update patient’s plan of treatment and provide physician with a status report on a regular basis. Maintain and update clinical records, according to agency policy, State and Federal guidelines. Complete paper and electronic documentation daily and timely to meet agency expectations. Responsible for scheduling clients on a weekly basis, updating as needed. Provide instruction and supervision of LPN's and HHA's on a timely basis; and in accordance with agency, State and Federal regulations. Must work a minimum of one weekend a month. Required to work one summer holiday and one winter holiday. The APRN or Physician Assistant is responsible for assessing physical and psychosocial status of patients in an inpatient and/or outpatient setting. The APPs will see GI and/or Hepatology patients in an outpatient clinic setting by means of interview, health history, physical examination, and diagnostic studies. This position could require inpatient M-F rotation on the UMH Hepatology inpatient service for the right candidate. For outpatient ambulatory work, this would require 7-8 half days in outpatient ambulatory clinics and at Uhealth locations. We have ambulatory practices at UHhealth Towers, UMHC Crohns and colitis Center, Kendall, Lennar, Doral, Sole Mia, Boca, Plantation, Deerfield sites. For Inpatient rotations, APP will round both independently and collaboratively on Hepatology Inpatient Service at University of Miami hospital no later than 8 am and have proper sign-out of patients with fellows at the end of the day and prior to weekends. Recognizes deviations from normal in the physical assessment. Works in collaboration with a physician in formulating treatment plans for health problems and follow-up. Writes prescriptions for medication, blood products based upon laboratory results, routine diagnostic and follow-up studies, therapeutic measures, and post-discharge care in accordance with written practice protocols. Accurately complete electronic medical records to include medical services rendered, test results, and supplies used. Monitor disposable inventory and place orders for medical supplies and pharmaceuticals as requested. Draw blood and process specimens for send-out. Provide patient education as directed by the provider.
