Provide care coordination for the beneficiaries assigned to the Medical Home Port Team utilizing evidenced-based clinical practice guidelines, regulations, manuals, reports and actively engages patients in their health care. Review details and expectations regarding referrals with patients. Provide support to patients and other healthcare professionals for patients receiving care services outside the MHP team to include assistance with making appointments, communication and information transfer between health care professionals, the patient and family (ensure knowledge/information transfer goes both ways between MHP team and patient) and assisting with problem resolution. Promote team awareness and patient safety by monitoring and tracking referral status and results, sharing such information and any subsequent plan of care with patients/family/MHP team and ensures that patients receive appropriate follow-up care and help in understanding results, treatment recommendations and plan of care. Promote team awareness and patient safety by monitoring and tracking the enrolled population for hospital admissions and emergent/urgent utilization, making contact with the patient, ensuring dissemination of relevant clinical information (treatment records and discharge summaries) and provides follow-up support and care coordination. Track and ensure smooth transitions by assisting patients and families as the patient moves from one care setting to another, such as from hospital to home and from one healthcare professional to another. Provide follow-up post referral, transition, or study. Provide care management services for the high-risk enrolled population. Link patients with community resources to facilitate referrals and respond to social service needs. Communicate test results and care plans to patients/families within established parameters. Assist patients in problem solving potential issues related to the health care system, financial or social barriers (e.g., request interpreters as appropriate, transportation services or prescription assistance). Be the system navigator and point of contact for patients and families, with patients and families having direct access for asking questions and raising concerns. May assume advocate role on the patient's behalf with the carrier to ensure approval of the necessary supplies/services for the patient in a timely fashion. Identify and utilize cultural and community resources. Establish and maintain relationships with identified service providers. Ensure that referrals are addressed in a timely manner. Remind patients of scheduled appointments via mail or phone. Ensure that patient's primary care chart is up to date with information on specialist consults, hospitalizations, ER visits and community organization related to their health. Provide logistical and clinical support to the patient, and family and healthcare teams. Identify patients with barriers to referrals/ transitions and helps the patient address them. Assemble information concerning patient’s clinical background and referral needs and (per referral guidelines) provide appropriate clinical information to specialists. Provide clinical care management services for high-risk patients.Provide relevant and individualized age specific care coordination for enrolled patient population.Provide a brief report to the MHP PCM or team with regard to patient needs and care coordination. Workweek runs from Sunday to Saturday. Includes a 30 minute lunch break. Rotational weekends and holiday coverage Job Specific Details and Tasks: Duties may include the following, dependent upon department:  Coordinate patient care in collaboration with a wide array of healthcare professionals. Facilitate the achievement of optimal outcomes in relation to clinical care, quality, and cost effectiveness. Ensure compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the MTF. Perform physical exam and health histories. Provide health promotions, counseling, and education. Administer medications, wound care, and numerous other personalized interventions. May provide clinical direction or assist in the instruction of care provide by other healthcare professionals within scope of clinical privileges or responsibilities, however, may not serve as Charge Nurse on a unit/ward.  Provide clinical direction for making patient care assignments based on the scope of practice and skill level of assigned personnel. Recognize adverse signs and symptoms and quickly react in emergency situations. Communicate and collaborate with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family and organization. Make referral appointments and arrange specialty care as appropriate. Perform nursing services identified in the TO.  Maintain a level of productivity comparable with that of other individuals performing similar services. Participate in peer review and performance improvement activities. Practice aseptic techniques as necessary. Comply with infection control guidelines to include the proper handling, storage, and disposal of infectious wastes, and the use of universal precautions to prevent the spread of infection. Function with an awareness and application of safety procedures. Perform efficiently in emergency patient situations following established protocols, remaining calm, informing appropriate persons, and documenting events. Anticipate potential problems/emergencies and make appropriate interventions. Notify supervisor, leadership, or other designated person regarding problems that the HCW is unable to manage. Apply an awareness of legal issues in all aspects of patient care and strive to manage situations in a reduced risk manner.  Participate in the implementation of the Family Advocacy Program as directed. Participation shall include, but not be limited to, appropriate medical examination, documentation, and reporting. Exercise awareness and sensitivity to patient/significant others' rights, as identified within the MTF.  Maintain an awareness of responsibility and accountability for own professional practice. Participate in continuing education to meet own professional growth. Actively participate in the command’s Performance Improvement Program. Participate in meetings to review and evaluate the care provided to patients, identify opportunities to improve the care delivered, and recommend corrective action when problems exist. Participate in the provision of in-service training to clinic/ward/unit staff members. Assist with providing training and/or clinical direction as applicable to supporting government HCWs (e.g., hospital corpsmen, students, etc.). Attend and participate in various meetings as directed. Perform timely, accurate, and concise documentation of patient care. Operate and manipulate automated systems such as ADS, MHS Genesis participate in clinical staff Performance Improvement (PI) and Risk Management (RM) functions, as prescribed by the Commander. Maintain DOD email account as directed. The HCW shall be responsive to all email and voicemail communications. Maintain documentation of all treatment provided in accordance with clinic directives and prepare such records and reports as may be required. All records and reports must be legible. Abbreviations must be only those listed in local instructions. Qualifications Minimum Qualifications: * Education: Minimum Associates in Nursing. * Experience: Minimum 2 years within the last 5 years in an inpatient pediatric ward setting. * Life Support Certification: Possess a current AHA or ARC BLS; PALS * Licensure: Must hold a full, current, and unrestricted nursing license in a State, Territory, Commonwealth, or the District of Columbia and must maintain a nursing license to practice in the United States of America. * Security: Must possess ability to pass a Government background check/security clearance. The salary range for this position is $43.12/hr - $86/hr. Compensation will be determined based on several factors including but not limited to skill set, years of experience, shift and the employee's geographic location.
