RESPONSIBILITIES: · Conduct UAS assessments for clients to determine their level of care and support needs. · Utilize the Uniform Assessment System (UAS) tool to document assessment results accurately. · Provide compassionate and professional interactions with clients in various community settings. · Submit completed assessments within designated timeframes and maintain detailed records. · Collaborate with other care providers to ensure a comprehensive assessment process. · Attend required training sessions and stay informed of assessment updates. QUALIFICATIONS Registered Nurse, currently licensed and registered to practice in New York State. UAS-NY Certified Bachelor’s degree in nursing preferred. Experience in Home Care, case management, or long-term home care. Demonstrated skills in planning, organizing and directing the work of others required. Experience in Intake or an admission team of PACE is highly desirable, including completion of UAS-NY Assessments. Proficient with DOH UAS assessment instrument.
