Job Description: The Triage Nurse conducts initial assessments to evaluate how urgently a patient needs care. They ask questions, check vital signs and coordinate care with other medical professionals, such as nurses and physicians. Duties & Responsibilities: The primary focus of a Triage Nurse is to ensure that the patients who are most in need of care receive it first and that other members of the care team are ready to begin treatments as quickly as possible. JOB PURPOSE: Conducting initial assessments and reassessments for members/clients for the elderly and disabled waiver program. KEY RESPONSIBILITIES: 1. Completes initial and annual assessments in client/member’s home and determines member ability to complete IADLs and ADLs. 2.
