I. Major Responsibilities: 1. Prepare for patient visits by reviewing schedules, checking for test results, and ensuring exam rooms are stocked and clean. 2. Escort patients to rooms, confirm visit reasons, document interviews, and assist with mobility as needed. 3. Take and record vital signs accurately, alert providers of abnormal findings. 4. Assist providers during exams, anticipate needs, and help maintain schedule flow. 5. Perform clinical procedures as ordered, including but not limited to: EKG, spirometry, immunizations, throat cultures, phlebotomy, and dressing changes. II. Working under the direction of the Practice Manager and/or Practice Coordinator, the LPN will interact with customers in person, by telephone and/or electronically. Since the role has significant impact on patient care, patient satisfaction, clinical operations and clinical revenue, the position requires outstanding customer service skills, attention to detail and having the appropriate technical skills to meet the job requirements in a timely and accurate manner. Monitor patient wait times and update patients/family in a timely manner. - Monitor medication stock to ensure appropriate supply of medication. Notify pharmacy and/or management team when appropriate. - Prepare exam room and appropriate procedure equipment in a timely manner. Assist physician for all procedures using sterile technique. - Telephone screening, including patient phone calls or messages (Epic Inbox, Patient Gateway or voicemail), results management, post-surgical follow-up, and addressing potential add-on patients. - Ensure that pathology, microbiology, virology, and other similar tests have been appropriately logged and followed-up per practice, department and hospital standards. - Assist with patient education including informed consents, wound care, medication and post-procedure instructions. - Prepare instruments, kits and trays for central sterile processing sending and receiving. Monitor instruments for any damage, notify management team when appropriate. - Prepare and process prior authorizations and disability forms. Follow-up in a timely manner. - Enter visit charges and diagnosis codes in Epic on physician’s behalf or as requested. Ensure that cosmetic procedure charges are entered in a timely and accurate manner. Overview $33/hour $5,000 Sign-on Bonus! ( available to external candidates only) Advocates is seeking enthusiastic, driven LPNs to offer medical support, care coordination and rehabilitation options to those in crisis. The Licensed Practical Nurse is responsible for the provision of direct care and nursing care to meet identified individuals’ health maintenance needs. This includes, but is not limited to, assessment, medication management, education and coordination of care. The LPN works as part of a multi-disciplinary team to support people at the program around all domains of living and to support the achievement of goals identified by individuals in Respite. This is a six-bed unit, therefore there are never more than six clients at a time. The day typically starts with rounding, morning assessment and medication pass. Throughout the day the LPN may be responsible for admitting and discharging clients from the physical health perspective which would include transferring medications, ensuring people have follow up with medical providers, VNA services/HHA services in place etc. Throughout the day clients at the program should be monitored, educated, and supported. Education should be provided via an individual and group basis. For example, maybe the LPN notices many people on the unit are struggling with sleep, so they offer a group education session on sleep hygiene. Maybe someone is admitted that has DM and the nurse provides some one-on-one education about when and how to check their blood sugar and make nutrition decisions. General health encouragement and education is of great benefit to the clients, for example maybe the LPN provides education around good nutrition choices and then the program makes that meal suggestion that night. There are set tasks and goals that need to be achieved daily, and there is wonderful room for the LPN to be creative in how they would like to provide health education and support!
