Monitors client acuity, census, staffing needs, and makes arrangements for coverage as needed, following established guidelines. Assures thorough, accurate documentation is completed for all clients on each shift and client assignment sheets are completed daily and tasks are carried out as assigned. Gives thorough report to oncoming staff, supervisors, and physician. Provides coverage in emergency services as needed. EDUCATION/EXPERIENCE/SKILL REQUIREMENTS: Graduation from an approved/accredited school of nursing. One or more years’ nursing experience within clinical area of service provided by facility preferred. Experience in behavioral health/psychiatric/addiction recovery environment preferred. Description Job Description PeaceHealth is seeking a RN Navigator for a Full Time, 1.00 FTE, Day position. The salary range for this job opening at PeaceHealth is $48.52 – $72.78. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc. Hiring Bonus may be available! Responsible for the coordination of patient cases for specific departments or service lines. In collaboration with the Medical Director(s), referring physician and interdisciplinary team(s), the RN Navigator coordinates disease-specific multidisciplinary conference; subsequent specialty consultations; and provides care as needed for patients and their families utilizing resources efficiently and effectively. Coordinates the development and performance of service line educational activities, program evaluation and outcomes research; acts as primary point of contact for service line patients. Th is a remote position for our new A-FIB fast track clinic – this position will need licensure in OR & WA – and will provide service system wide for this patient population Essential Functions Patient and Family Liaison - Act as a single point of contact for patients and families including telephone triage regarding service line symptom management. Assist patients and families in understanding their diagnosis, treatment options, and the resources available, by utilizing excellent interpersonal skills. Act as a liaison and primary point of contact between surgery team and surgical patients and families before, during and after surgical procedures to include scheduling of follow up appointment(s), giving discharging patients specialized instructions for surgical preparation, home care, lab requisitions and prescriptions. Perform post-surgery phone calls to evaluate care progress and answer any questions. Clinical Liaison - Collaborate with the service lines and notify surgeons and surgical team if there is a critical patient who requires stand-by preparation for surgery or specialized care. Collaborate with all disciplines (dietary, cardiac rehab, pharmacy, home health, etc.) to ensure that the patient is discharged in a timely manner; monitor inpatient length of stay (LOS) and collaborate with the various disciplines to expedite timely discharge. Reconcile all medication for the surgeons/PA’s and prepare all discharge instructions. Clinical review - Develop and maintains clinical guidelines, policies, protocols and documentation for practice in service line programs. Make rounds in service line surgical cases. Collaborate with staff nurses, review labs, write information for the surgeons and PA’s on the progress notes and report any abnormal findings to the surgical team that may need to be addressed. Perform chart review and collection of data required by the state and other entities regarding cardiac surgery outcomes. Educator - Develop patient and clinician education program, tools, and documentation. Reinforce patient education and direct patients and families to available resources and supportive services. Collaborate with nursing education regarding competencies. Facilitate a class for service line patients bimonthly. Provide preoperative teaching to inpatient “add-on” cases as needed. Ensure patients receive a treatment plan that is understandable and feasible and meets their specific needs, and coordinate timely scheduling of tests, pre-surgical procedures, specialty appointments and treatments, including initiating appropriate referrals (both internal and external) to community resources. Follow patients through the continuum of care, in collaboration with key stakeholders in the treatment plans. Assess for clinical, emotional, spiritual, psychosocial, financial and other patient/family needs. Coordinate multi-disciplinary planning conferences and education for service line programs; develop concise patient summaries for use by the care team and document recommendations made utilizing standardized care protocols in accordance with nationally recognized care guidelines. Collaborate and follow-up with the multidisciplinary heart failure team. Along with leadership guides appropriate service line program growth that is in accordance of governing bodies and best practice. Facilitate quality improvement initiatives, assist with the development of applicable metrics. Performs other duties as assigned. Qualifications Education Required: Degree from an Accredited School of Nursing Preferred: Bachelor of Science in Nursing or Preferred: Master's Degree Experience Minimum of 5 years nursing experience with 3 years nursing experience caring for CV patients in acute or outpatient setting (e.g. Cath Lab, CVICU, EP, cardiac stepdown, ambulatory cardiology, etc.) Management and/or program development experience desirable. Must be able to work in a self-directed environment. RN licensure in WA and OR required upon hire and candidate must be able to receive AK licensure as requested. RN - Psychiatry and Behavioral Medicine Unit (0.9 FTE, Nights) We are looking to hire .75-.9 FTE Night Shift RN's for our Psych and Behavioral Medicine Unit (PBMU)! No pediatrics experience? No problem! If you have experience working with adult populations, we will provide training on working with pediatric patients and their families while leveraging the adult RN experience you already have. We will personalize an orientation just for you to set you up for success in the pediatric specialty you are hired for. The Psychiatric and Behavioral Medicine Unit (PBMU) consists of 41 beds. Care is provided on a 24-hour basis, seven days a week. The PBMU is committed to the evaluation and behavioral stabilization of children (ages 4 through 17) with emotional, behavioral, and neuropsychiatric disorders. Depression, developmental delays, attention deficits, anxiety, pervasive developmental disorders, conduct disorders, oppositional-defiant disorders, anorexia/bulimia, and psychosis are among the disorders commonly diagnosed. The unit is also invested in treating children with unique concomitant problems such as hearing impairments, chronic medical illnesses, or physical developmental disabilities. The PBMU program utilizes a model of care that emphasizes an interdisciplinary approach. Registered nurses; child and adolescent psychiatrists; psychiatric nurse practitioners; psychologists; mental health therapists; social workers; pediatric mental health specialists (PMHS); special education teachers; occupational, recreational, art, music, and speech therapists; and students from these various disciplines all participate in the development of individualized treatment plans. Care includes the provision of behavior management in a milieu setting; family/caregiver and patient education; group activities; supportive family/caregiver and individual support; psychotropic medication management; and limited medical nursing services. As an integral member of the Health Care Team, the RN functions within the Scope of Practice as defined by the State, the ANA Scope and Standards for Pediatric Nursing Practice, and adheres to Seattle Children's policies/procedures and guidelines of care. Responsible for providing safe, family centered patient care.
