. The Division of Family and Child Health (DFCH) of the New York City Department of Health and Mental Hygiene is charged with the creation and oversight of programs, policies, services, and environments that support physical and socio-emotional health, and promote primary and reproductive health services, health equity, social justice, safety and well-being for New York City families and children. DUTIES WILL INCLUDE BUT NOT BE LIMITED TO: --Answer and return all calls to the Diabetes Medication Administration Form (DMAF) Hotline. --Provide the school nurse with current approved DMAF orders in a timely manner. --Collaborate to coordinate care between the OSH providers, endocrinologists, parents, DNEs, and school nurses for the student with diabetes DMAF approval according to OSH policy. --Update nursing's DMAF Hotline log daily. --Provide approved DMAF to OSH and agency nurses who may not have access to the Automated Student Health Record (ASHR). --Contact endocrinologists for clarification of DMAF orders. Reach out to nurses and parents/guardians for signed consent or missing information from DMAF to expedite DMAF approval. Liaise with the OSH medical unit for status and updates on DMAF approvals. Ensure DMAF orders for NYC school children are approved expeditiously and are in ASHR. RN Supervisor

Laconia Nursing home is an independent, family-owned facility in the Bronx. We are looking for an RN Supervisor to join our leadership team. Essential Duties: Establish a compassionate environment by providing support to Residents and their families. Maintain documentation of Resident care services by auditing Resident and department records. Create appropriate care plans for residents Handle all supervisory duties for assigned units, including overseeing LPNs and CNAs that report to you Help establish and implement employment policies and procedures Maintain the standards of care for the units Review Resident records & quality of care Monitor overall care & review individual Residents’ cases Address questions or complaints brought forward by Residents or their families Education, Experience and/or Training: Long-term Care experience preferred Valid RN license required 2-4 years management experience. knowledge of City, State and Federal regulations pertaining to licensed skilled nursing facilities. Overview Good Shepherd Hospice is looking for passionate, dedicated RN to provide End of Life care to patients at home on weekends. 8:30AM to 4:30PM Weekend shifts Job Details Our Hospice RN’s: The Community Hospice RN is responsible for providing nursing care to hospice patients, residents and/or family within their home setting, focusing on symptom management and pain control. The ideal candidate will have knowledge of hospice concepts, philosophy, policies and procedures. Prior experience in home care, hospice and/or oncology preferred. Qualifications: Current New York State Registered Nurse (RN) license Minimum of one year of recent hands on experience Basic Life Support Certification Current, valid NY State driver's license and own transportation. Salary Range USD $48.00 - USD $64.00 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
