RESPONSIBILITIES OF THE ADON: Ensure that all nursing personnel in our facility comply with established standards, practices, and regulatory requirements for the highest quality of resident care. Support the development and implementation of nursing department budgets in coordination with the facility administration. Is responsible for surveillance, rounding, analyzing, interpreting and the reporting of healthcare-associated infections Supports the development of evidence-based organizational infection prevention policies/standards that meet state, federal, regulatory and accrediting agencies standard Leads in infection prevention and serves as a resource, educator, researcher and proponent of change in order to influence infection prevention improvement to provide quality health care within the organization REQUIREMENTS OF ADON: As the Assistant Director of Nursing, you must possess strong interpersonal communication skills and the ability to deal effectively with patients, families, physicians, community resources, and the general public. Associate degree in Nursing or higher from an accredited school of nursing. Must have a valid New York RN license Minimum 3 years’ experience as an ADON or a previous DON role preferred. MDS Coordinator

Laconia Nursing Home is seeking an experienced full-time RN MDS Coordinator. The primary purpose of this position is to coordinate the RAI process assuring the timeliness, and completeness of the MDS, CAA’s, and Interdisciplinary Care Plan. Responsibilities of MDS Coordinator: Monitors and guides the completion of assessments in a timely manner Involvement and supervision of inputting of MDS assessments, the transmission to the state, and obtains receipt of the validation Schedules and facilitates care plan conferences Facilitates monthly QM meetings Manages Care Planning Process Qualifications of MDS Coordinator: Must hold a Nursing Degree from an accredited college or university A current, unencumbered, active RN/LPN license and CPR At least 2 years’ experience in Long Term Care as an MDS Coordinator. Skills and Qualities of MDS Coordinator: Outstanding knowledge of Case-Mix, Federal Medicare PPS process, and Medicaid reimbursement. Excellent written and verbal communication skills and critical thinking abilities, Solid understanding of current federal, state, and local standards, regulations, and guidelines that govern our facility Ability to organize and prioritize.
