This contracted position is located in the Gallatin County Sheriff’s Office Detention Division and is responsible for providing professional medical services to inmates within the Detention Center. The role ensures inmates receive appropriate care in accordance with federal, state, and local laws, Montana Jail Standards, and recognized correctional healthcare and community standards. The position is supervised by the Detention Center Clinic Director and collaborates with a multidisciplinary team of medical professionals, detention staff, and community healthcare providers. Essential Duties and Responsibilities Provide professional medical services within the Gallatin County Detention Center, including diagnosis, treatment, and medication management. Ensure all care is delivered in compliance with established laws, regulations, policies, and professional standards. Provide direct patient care in the Detention Center clinic with a minimum of 12 hours per week, 52 weeks per year, except for approved time off. Be available “on call” by County-issued phone during clinic hours, including weekends and holidays, unless time off is approved. Communication may include text, phone, or email. Respond to special circumstances beyond regular clinic hours as needed (e.g., infectious disease outbreaks, emergencies, disasters, or legal obligations). Participation may include on-site care or conference calls. Follow safety and security protocols specific to working in a detention setting. Collaborate with clinic staff to ensure continuity of care and coordinate referrals to community providers, specialists, hospitals, and emergency services. See inmates as scheduled by clinic staff and therapists; communicate professionally and respectfully with inmates, detention staff, community providers, and family members as appropriate. Review and sign assigned documents in the electronic health records (EHR) system; maintain timely and accurate documentation of all patient encounters. Develop care plans, schedule clinic follow-ups, and conduct discharge planning as clinically indicated. Maintain active professional licensing and certifications required to provide services in the State of Montana. Knowledge, Skills, and Abilities Principles, practices, and laws governing professional nursing and/or medicine. Medical terminology, diagnostic and preventive techniques, pharmacology, immunizations, and communicable disease intervention. Standards of correctional healthcare, inmate rights, and Detention Center policies and procedures. Documentation and medical records management, including use of electronic health records. Professional standards for patient confidentiality and HIPAA compliance. Ability to: Apply medical knowledge in varied and complex patient situations. Make independent medical determinations under pressure. Communicate effectively, both verbally and in writing, with diverse individuals. Master’s degree in nursing from an accredited program. Current Family Nurse Practitioner (FNP) certification. Two (2) years of professional related experience. Prescriptive authority and active licensure with the Montana State Board of Nursing. Mental health experience preferred. Prepares prescription orders/inquiries/renewals to facilitate patient care by writing orders for drugs and controlled substances and relaying information to pharmacy. Implement interventions consistent with the established plan of care in a safe and appropriate manner. Apply established therapeutic practices to patient's problem. Prescribe medications according to Controlled Substance Registration Certificate issued by the DEA. Generates referrals to appropriate services as needed to further diagnose and/or progress of care. Administers medication and immunizations to ensure patient health by injection, inhalation or oral methods. Perform patient consultations/physical exams to determine the assigned patient's needs independently and under physician direction. Includes obtaining patient medical history, checking vital signs, performing tests relevant to patient condition and recording findings.
