Nurse Practitioners see patients independently and collaborate with physicians, depending on the patient’s specific situation. Nurse Practitioners provide longitudinal care and build meaningful patient relationships; this creates trust and helps our patients understand and feel connected to their care plan. Nurse Practitioners practice in our outpatient practices on a collaborative care team composed of a physician, nurse practitioner or physician assistant, medical assistants, a dedicated medical scribe, and support from registered nurses and care managers. We partner with a network of elite specialists and hospitals for specialty and acute care. As such, our healthcare providers can focus on care within the clinic: primary care, care coordination, and population health. Core Responsibilities: Provision of exceptional primary care. Conduct office visits for routine and acute issues. Administer Annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation. Care coordination with other providers, specialists, testing facilities, and agencies. Population health leadership, in coordination with the Care Team (e.g., making sure all eligible females get their evidence-based breast cancer screening every 2 years) Assisting the care team with phone triage and outreach. Educating patients on their health conditions, care plans, and treatments. Participating in Oak Street Health promotional activities. Conducting home visits as needed. Other duties, as assigned. This role reports to the Center Medical Director and works closely with operational leadership. Maintains extensive knowledge of their specialty literature including awareness of developing areas. Consistently derives correct diagnoses through synthesis of clinical data and proper selection and interpretation of diagnostic studies. Develops comprehensive differential diagnoses for patients. Revises the plan of care to meet the changing needs of the patient. Prioritizes work and assists staff to provide prompt and efficient client flow. Consistently maintains high productivity according to identified standards. Leads/participates in projects to improve productivity. Appropriately performs medical procedures consistent with departmental guidelines, privileging, and clinical roles/responsibilities. 2. Education- Appropriately and professionally educates clients and/or families regarding medical condition, treatment, and follow-up. Collaborates with clinicians, staff, and students to promote continuity and patient/family centered care. Develops educational materials based on learning needs of clinicians, staff, and students. Provide education/instruction to physicians, housestaff, nursing, ancillaries and/or students. Actively recognizes and participates in identification and achievement of self-learning needs. Responsibilities The Nurse Practitioner delivers annual risk assessment in a residential setting or telehealth, within the scope of practice for a Nurse Practitioner, as delegated by the Collaborative Physician. Essential Duties and Responsibilities Conduct comprehensive in-home health risk assessments to identify all active and chronic disease conditions, as well as determine all physical, mental, and social needs present at the time of the visit Takes history, examines, determines diagnoses. Provides written documentation of patient visit, per NCQA standards Takes patient vital signs, as necessary. Places case management referrals and communicates with PCP as necessary. Communicates with patients, caregivers, agency nurses, other providers and vendors as necessary to assure proper diagnosis. Performs all clinical duties while observing OSHA Universal Precautions Maintains patient confidentiality Attends required meetings and in-services and participates in committees, as requested Participates in professional development activities and maintains professional licenses and affiliations In this role you may work with. . . Adaptability to differing weather conditions and patients’ home/residential environments Full range of body motion including handling/lifting patients.
