Completes assessments of psychosocial clinical needs for patients in relation to social, psychological, financial, and family situations. Job Specific Duties Prioritization of Patients in the ED Collaborates and communicates with ED Care Team members to identify patients in need of psychosocial assessment/interventions and/or immediate discharge needs; Prioritizes workload between crisis patients, patients in need of routine interventions, and patients with non-critical (i.e. home health, transportation) discharge needs; Documents interventions and progress notes in Midas Discharge and Transitions Planning Addresses discharge needs and discuss options for discharge from the ED and/or alternative care plans with the ED staff, patient, family/representative, and other care team members as needed to ensure a safe discharge; Collaborates with ED Care Team to determine clinical post-service needs (e.g., skilled nursing facility, long term acute care, home health, durable medical equipment, transportation); Locates community/alternative resources that meet clinical and, if applicable, psychosocial needs of the patient; For patients with psychosocial discharge needs: Coordinates psychiatric or addiction services. Provides resources for patients at risk for abuse, neglect, or domestic violence. Identifies resources for patients who are homeless: Information about area shelters, Clothing and referrals to food pantries, Referrals to Healthcare for the Homeless; Collaborates with Primary Care Physician. Collaborates with the patient/family regarding the discharge plan and ensure understanding of the resources provided Crisis Intervention Serves as a patient advocate. Collaborates with community outreach agencies and coordinate resources to prevent inappropriate future use of the ED; Coordinates activities to secure a PCP for the patient Intervention Implementation Identifies possible or actual patient/family crises, provides crisis intervention and emotional support and creates and facilitates effective plans for resolution; Serves as organizational contact for first responders, law enforcement and community agencies to assimilate information related to possible abuse, neglect, domestic violence or inadequate home situations for patients brought to the Emergency Department. Continuing Care & Discharge Planning Identifies/communicates psychosocial and discharge planning needs of the patient upon referral and throughout hospitalization; Sets priorities in planning/organizing timely interventions, continuing care and discharge plan based on patient, family, community resources, and health care team information Escalate Cases as Necessary Escalates barriers according to escalation process; if the barrier is clinical in nature, refer to Physician Advisor and escalate to the CM Manager for all other barriers Positive Relationships Maintains cooperative working relationships with patients, families, heath care team members, community health and social agencies. Reaches out to community to learn of and utilize new resources for meeting patient needs. Assists patients and families with obtaining community assistance by referral to proper resources.
