The Area Clinical Specialist LPN/RN is responsible for assisting in onboarding and training new Wellness Directors. This individual will support the following states: MA, MD, and NY. Responsibilities : Involves being completely familiar with the state regulations governing Assisted Living and ensuring compliance in department by regularly reviewing regulations checklist. Performs timely and accurate resident assessments utilizing assessment tool, and communicating changes to the Executive Director for billing purposes Manages expenses within given budget parameters utilizing spend-down sheets. Provides oversight with medication management to assure all MAR's are correct and that the residents receive medications timely and accurately. Working with consultant pharmacist as needed to assure community is in compliance. Ensures all charting is performed in a timely manner and in accordance with state regulations. Provides timely and honest communication with families in order to promote good relationships and to keep them informed about the resident's condition and community events. Provides marketing support within the community by participating in tours and discussions with potential residents and their families about clinical needs of the prospect. Provides assessments for potential residents to determine their eligibility for Assisted Living. Creates positive team member relations through staff appreciation programs, recognition of excellent performance, and retention through regular feedback systems and promotional incentive programs. Provides appropriate guidance for team members by planning work schedules, assigning work duties while on shift, and directing the work on a daily basis. Provides on-call backup to community when not on duty. Maintains confidentiality of all pertinent personal or health information concerning residents and staff. Ensures that any incident reports are completed properly and promptly. Other responsibilities as assigned by the Regional Clinical Manager Qualifications: Qualified candidates must be currently licensed as a RN or LPN with a minimum of 1-year experience in senior housing and/or geriatric nursing. Minimum 2 years’ experience working as a Director of Nursing or Wellness Director with a demonstrated success in leading nursing teams to meet successful resident outcomes. Candidate must be able to travel to various communities within the assigned market. Prior management experience preferred. Candidate must either be certified or be able to become CPR and First Aid Certified within the first 90 days of employment. Candidate must also be able to read, write and speak the English language and be able to communicate effectively with residents, families, physicians, and team members. Responsibilities Assist with injections, pap smears, biopsies, and LEEP procedures. Prep lab specimens for send out. Assess patients for changes in condition, including but not limited to abnormal labs, x-rays, scans. Set up procedures (i.e. port placement and blood transfusion), referrals, and obtain prior authorization. Obtain pathology reports, manage faxes, and return emergent and non-emergent patient phone calls. Maintains efficient and appropriate patient flow. Documents all elements of care. Serve as a resource to the Medical Assistants. Communicate all pertinent information and any interventions to the physician, nurse, or advanced practice provider Assess patient and family needs. Complete new patient chart prep for physician team ensuring all elements of the patients work up is available to for review upon first appointment with provider. Qualifications High school graduate or equivalent. Graduate from an accredited school for practical nursing. Current LPN state license and registration with the State Board of Practical Nursing in practicing state. Minimum of two years-experience as a practical nurse in a hospital or clinic setting preferred but not required. Current CPR certification required. Pediatric experience preferred. Ability to travel. Must provide record of a completed diploma (or equivalent) or academic transcript for those areas performing complexity testing. WORKING CONDITIONS Clinical patient care environment with minimal exposure to hazards such as excess noise, dust, or extremes in temperature. Frequent exposure to communicable diseases, toxic substances, medicinal preparations, body fluids and other conditions common to the healthcare environment.
