Responsibilities

  • Verify plan of care with the RN prior to each visit.

  • Collaborate with physician and RN frequently to ensure patient needs are being met; Notify them as soon as possible when patient condition changes.
  • Teach patients and caregivers self-care techniques.
  • Check vital signs.
  • Review all medications and perform a medication reconciliation at each visit.
  • Perform specialized procedures ordered by physician (i.e. foley catheter change).
  • Prepare equipment and materials for treatments using the aseptic technique.
  • Complete documentation timely and accurately.


Qualifications

  • One year of clinical LPN experience.
  • Current LPN license, specific to the state(s) you are assigned to work.
  • Current CPR certification.
  • Valid driver's license, reliable transportation and liability insurance.

Newport News, Virginia Overview Performs duties under the direction or supervision of a licensed medical practitioner or a registered nurse. Responsible for conducting a focused assessment of the patient to include vital signs and gathering data, implementing established plans of care, and evaluating effectiveness of treatments in the medical practice setting. May float to other practices within the region providing operational support as needed. What you will do Accurately completes all required clinical documentation consistently per Riverside Medical Group standards. Maintains documentation for quality assurance to include but not limited to point of care testing, medication logs and annual wellness visits. Works with providers to assure care gaps are routinely addressed with each patient. Documentation is accurate, legible and completed same day. Knowledgeable and skillful in lab functions and lab proficiency testing. Maintains excellent clinical competency by adhering to the standard clinical expectations for Riverside Medical Group. This includes updating, verifying, and maintaining patient records by properly interviewing the patient and confirming purpose of visit. Prepares patient for examination by obtaining vital signs proficiently (including manual vital signs when mechanical devices are not available).
