Communicates essential information to appropriate parties. ⦁ Makes rounds on assigned residents at least once every two hours. ⦁ Collaborates with Care Plan Coordinator to develop and maintain nursing care plans and update these plans on a regular basis and as the resident's condition changes. ⦁ Attends and participates in interdisciplinary resident care conferences as requested. ⦁ Assists the attending physician with rounds and special procedures as needed. ⦁ Participates in the yearly written evaluations of nursing assistants and documents accurately their strengths, weaknesses and the steps that need to be taken to improve their performance. ⦁ Assists Staff Development with orientation and on-the-job training for new nursing personnel as needed. ⦁ Collaborates with nursing supervisor in documenting disciplinary concerns for nursing assistants on designated shifts, including reprimands, oral and written warnings, and corrective action recommendations. ⦁ Notifies nursing supervisor, resident’s physician, and family of a change in the resident’s condition or an accident/incident involving the resident. ⦁ Assumes any duties that are assigned by the RN Supervisor/ADNS to provide optimal quality resident care. ⦁ Performs other related duties and attends meetings as may be deemed necessary by Supervisor / Administrator and or Director.
