Provide Compassionate Care Where Patients Feel Most Comfortable

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our Home Care team. In this role, you will provide direct nursing care to individuals and families in their homes and community settings, helping patients maintain independence and achieve the best possible health outcomes.

Working under the guidance of the Nursing Supervisor and in collaboration with the patient’s primary nurse, you will play a vital role in delivering patient-centered care, monitoring health conditions, and supporting patients and their families through compassionate service.

Key Responsibilities

Provide direct nursing services to patients in home and community environments.

Work collaboratively with the Nursing Supervisor and primary nurse to implement individualized care plans.

Perform patient assessments, monitoring, and documentation of care provided.

Support patients and families with education and guidance related to healthcare needs.

Maintain accurate documentation and comply with privacy and confidentiality standards.

Communicate effectively with patients, caregivers, and healthcare team members to ensure quality care.

Minimum Qualifications

Education

Graduate of an approved Practical or Vocational Nursing program.

Experience

Minimum of two years of medical or surgical nursing experience strongly preferred.

Knowledge And Experience In Community Health Nursing Preferred.

Home care or home health experience preferred.

Licensure and Certification

Current Massachusetts Licensed Practical Nurse license.

Basic Life Support certification for Healthcare Providers required.

Proficiency with computer systems and electronic documentation.

Ability to maintain confidential patient information.
