About the unit We are a 35 bed Patient Family Centered Care ( PFCC ) Rehabilitation Unit designed for patients with Intensive therapy needs. Our patient's average length of stay is between 12 –14 days. We provide a transitional setting between the hospital and the patient's home. It is ideal for those who are medically stable, motivated and able to physically tolerate three hours of therapy per day, five days per week. Our team includes patients, their families, and other medical and case management professions. We have a team approach for our patients individualized treatment plan. About the unit 9 South is a 32 bed medical unit that cares for a wide variety of diagnosis and multiple chronic co-morbidities in the patients with an age range 18-100+. On our unit houses our ACE (Acute Care of the Elderly) program; This program helps to avoid complications for our patients 65+ so that they can return to their baseline function prior to their hospitalization and hopefully return to the home setting. The unit also sees a variety of medical overflow so you will never be bored. We have the capability to monitor 8 telemetry cardiac patients. Regularly re-evaluates patient nursing needs. Initiates the plan of care and makes necessary revisions as patient status and needs change. Uses health assessment data to determine nursing diagnosis. Provides health care instructions to the patient as appropriate per assessment and plan of care. Visit clients routinely in order to observe and evaluate each clients’ physical and emotional status. Maintain the confidentiality of all client care information. Monitor nursing care to ensure that all clients are treated fairly, and with kindness, dignity, and respect.
