Telephonic Nurse Case Manager

$84,000 - $90,000 / year
We are seeking a Telephonic Nurse Case Manager with compact and NY RN license to work Monday - Friday, 8 AM to 5 PM Eastern time zone. CCM strongly preferred. The Telephonic Case Manager provides remote medical case management services to injured individuals, many of whom were industrially injured. A Telephonic Case Manager assists consumers in their recovery so that they may return to the highest level of function as possible. Medical services are coordinated and assessed frequently, vocational options are explored with the injured worker and their employers, and pre-injury employment or alternate employment is secured. The Telephonic Case Manager works with all parties to the claim including: the injured person, the claim’s examiner, employer, attorneys (plaintiff and defense) and medical providers. Bilingual Field Nurse Case Manager - Austin and San Antonio, TX

Paradigm is looking for a full-time, 0.75 - 1.0 FTE (30-40 hours per week), benefited Bilingual (Spanish speaking) Field Nurse Case Manager to cover the Austin and San Antonio, TX, areas . This is a salaried position: base salary range at 0.75 FTE is $66,000-$71,250. At 1.0 FTE, base salary range is from $88,000-$95,000 annually. A Field Case Manager role is a home-based position with travel, up to 2 hours one-way (4 hours round trip), to doctors’ offices, hospitals, and various other locations. This individual is responsible for the medical case management of work-related injuries, which includes assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager works with insurance carriers, medical care providers, attorneys, employers, and employees, and closely monitors the progress of the injured worker and reports results back to the employer and insurance carriers.
