Position Summary: Assist physician with duties necessary to support the needs of the practice working within and to the maximum scope of license Education, License & Cert: LPN ‐ Must be licensed in the state of practice. Directs patient to exam room and prepares patient for exam or treatment by identifying height, weight, respirations, blood pressure, temperature, additional testing, and records same on chart. Basic knowledge of surgical instruments 12. Prepare surgical trays as needed for procedures for the day. 13. Maintain infection control measures including but not limited to proper cleaning of surgical equipment and use of autoclave 15. Proficient with autoclave sterilization process 16. Knowledge and proficiency of dressing changes and suture removal 17. Assess signs and symptoms of infection and reporting to surgeon and/or provider 18. Summary Care Coordinator/Wellness Nurse-Per Diem will assist in coordinating the medical management of patients, using an outcomes-based approach. In collaboration with other members of the healthcare team, the Care Coordinator identifies at risk patients, develops a plan of care, and coordinates care with the patient’s involvement. In addition, this position will also support health promotion and illness prevention for Medicare patient though completion of the Medicare Annual Wellness Visit. Position Summary: The Pre-Admission Registered Nurse (RN) performs a verbal assessment with the patient prior to admission via a phone interview or in-person visit, as well as collects the patient’s medical history from previous medical records. The nurse also works closely with the anesthesia department and the Prep/ Recovery Department (PRU) in coordinating the patient’s needs. The RN cares for a varied case load of surgical and procedural patients, making advanced assessments of the patient’s physical and psycho-social needs. She/he will be assigned to provide service to patient populations in Robert Packer Hospital. She/he is skilled in physical assessment related to peri-anesthetic care which includes respiratory management, pain management, and patient teaching. She/he is skilled in physical assessment related to pre-surgical state of a patient as well as the patient’s assessment post-operatively. She/he is well-versed on the post-op teaching necessary for the above-named patient populations. Our facility sits directly on the local bus route, offering a bus stop right at Elcor for easy commuting! Responsibilities This position manages the daily clinical regimen of each resident. LPNs implement and evaluate the care of residents ensuring that the highest degree of quality care is always maintained. Assists in planning and implementing the nursing care of residents within the skilled nursing setting with the supervision of a Registered Nurse Responsible for ensuring continuity of care of the residents between shifts by providing direct care as well as supervising the care given by CNAs and supportive staff members. Maintains role as resident advocate with a focus on the facility’s mission. Documents accurately per policy on residents in a change in condition or on 24 hour report, on an ordered documentation such as behavior, pain, sleep, functional mobility, stop and watch, Medicare look back as well as required admission, lateral transfer, or discharge documentation. Observes for changes in the resident’s condition and suggests nursing needs. Obtains vital signs, lung sounds and other needed clinical data, notifies on duty facility supervisor and medical on any new changes in condition with also having appropriate documentation in resident record so that appropriate action can be taken Ability to adequately monitor, treat and report resident pain whether physical or emotional, verbal or nonverbal. Demonstrates knowledge of symptoms of pain and depression. Utilizes appropriate pain management techniques checks for available prn’s and administers appropriately as well as monitors for and documents/reports effectiveness. Administers medications safely and per policy, follows the medication rights and reduces the potential for medication errors. Demonstrates competency in medication administration per regulations. Utilizes prn medication when needed.
