Responsible for security of employees and residents on assigned shift. Acts in administrative capacity in absence of the Director of Nursing, Long Term Care and Assistant Director of Nursing, Long Term Care as assigned. Completes the initial resident profile and updates as necessary according to the care plan. 9. Implements care and services that recognize age and diversity specific needs and issues of customers and residents served. Maintains an ongoing rapport with family members or designee. 11. Receives reports on residents from the staff nurse and conveys pertinent resident information to IDT as indicated. 12. Reviews the condition of residents with the Clinical Care Coordinator and communicates pertinent information to physicians involved with resident care. Calls family regarding condition changes. 13. Works with Unit Coordinator on referrals to clinics and other physicians as needed. Participates in orientation of new employees. Collects quality review studies and participates in Quality Assurance program. 16. Handles all situations with professionalism, tactfulness, politeness, and is always courteous to residents, visitors, and other employees. 17. Regularly confers with Director of Nursing, Assistant Director of Nursing, and Clinical Care Coordinator. Participates in meetings and committees as requested. Consistently works to maintain staff morale and promotes their working together as a team. Interacts with staff from other disciplines and areas in a caring and professional manner. Interprets the nursing care plan to nursing staff as assigned. 21. Delegates responsibility to nursing personnel for the direct nursing care of specific residents during each shift of duty. 22. Assists in scheduling duties as needed. Assists the unites with emergent situations and follows through with all new orders. 24. Responsible for the supervision of the total nursing activities on his/her shift. 25. Evaluates and performs nursing care per physician's orders, and in compliance with facility policy and procedure plans. Assists with medication pass and treatments as needed. Explains procedures and treatments to residents commensurate with their level of understanding. 28. Consistently administers treatments, applies various apparatus, uses equipment safely. Lifts, positions, transfers, and transports residents using proper body mechanics and available equipment. Assists the physician with diagnostic and therapeutic procedures. 29. Utilizes sound nursing judgment in emergency situations and demonstrates nursing technique and procedures and assists and supervises non-professional personnel in delivering resident care. 30. Always respects resident's rights as written in the Taylor Health Center Resident Rights and Responsibilities Manual. Follows infection control in accordance with all applicable facility policies. 32. Performs documentation in compliance with facility policies and procedures. 33. Works with the Clinical Care Coordinator to record resident's needs and problems on NCP and revises as appropriate through evaluation of resident's response to nursing care. 34. Provides reports on incidents and accidents of both residents and employees and follows through with notification to appropriate personnel. Implements any changes to POC and documents accordingly. 35. Practices in accordance with the limits of the New York State Nurse Practice Act, and facility policies and procedures and nursing standards of care. Displays a positive attitude towards work assignments, residents, and co workers. Completes annual competencies per job description in a timely manner. Performs other related job duties as assigned or requested by the Director of Nursing or Assistant Director of Nursing. Required to work weekends, holidays and various shifts as needed.
