Position Details Primary Department or Practice: UHS Palliative Care ----- Position Summary: We are seeking a compassionate and experienced Nurse Practitioner or Physician Assistant to join our Virtual Palliative Care team. This role provides high-quality, patient-centered care to individuals with serious illness through telehealth platforms. The NP/PA will work collaboratively with an interdisciplinary team to support patients and families in managing symptoms, understanding their illness, and aligning care with their goals and values. Key Responsibilities: Conduct virtual palliative care consultations and follow-ups via secure telehealth platforms. Perform comprehensive assessments including physical, emotional, psychosocial, and spiritual needs. Develop and implement individualized care plans in collaboration with patients, families, and the care team. Provide symptom management, advance care planning, and goals-of-care discussions. Coordinate care with primary care providers, specialists, and other members of the healthcare team. Document all patient interactions in the electronic medical record (EMR) in a timely and accurate manner. Participate in interdisciplinary team meetings and contribute to continuous quality improvement. Educate patients, families, and caregivers on disease progression, treatment options, and supportive resources. Qualifications: Current licensure as a Nurse Practitioner or Physician Assistant in NY Minimum of 2 years of clinical experience in palliative care, hospice, geriatrics, or chronic disease management. Experience with telehealth platforms and virtual care delivery preferred. Strong communication, empathy, and interpersonal skills. Ability to work independently and collaboratively in a remote environment. Work Environment: Fully remote Requires reliable internet connection and private workspace. Occasional travel for team meetings or training may be requested.
