Job Description Position Summary: Provides personal care and assists with activities of daily living and homemaking to Long Term Home Health Care patients in their homes following a care plan developed by a Primary Case managing RN.. Is responsible for following the aide care plan, reporting issues related to providing care and patient symptoms as directed in the plan. Education, License & Cert: High School Diploma or GED Required Completion of a NYS DOH approved certification program and is in good standing Experience: Minimum one year of relevant experience. Experience with public contact is preferred. Essential Functions: 1) Updates Knowledge and documentation per agency standards Understands and adheres to GCMC’s compliance standards as they appear in GCMC’s Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy. Attends in‐service programs designed to update knowledge and skill related to patient care and to keep abreast of all pertinent federal, state and GCMC regulations, laws, and policies as they presently exist and as they change or are modified relevant to job Reads aide care plan at the beginning of each home visit and performs all those tasks listed on care plan and follows policies of LTHHCP in the delivery of care.
