Services are provided based on the ages of the population served -- adult through geriatric, 65 years or older -- and in collaboration with the multidisciplinary care team. Demonstrates the skill necessary to meet the needs of all residents served in Skilled Nursing through support of the nursing process; collects resident care data, reports findings, assists team in meeting resident care goals. The Certified Nursing Assistant SNF is under the supervision of the Skilled Nursing licensed staff and therapists and reports to the Skilled Nursing Director or designee.
