Overview NaphCare is hiring a Registered Nurses (RNs ) to join our team of medical professionals in Tacoma, WA, where we manage all aspects of healthcare for the individuals who are currently incarcerated in the Pierce County Jail As a Correctional Registered Nurse (RN ), you will work alongside correctional officers and other medical professionals to provide care to the individuals in residence. Working within a controlled environment, where security is top priority and the days are never dull, you will have various duties to keep you engaged including performing physical assessments, administering medications, managing chronic illnesses, and responding to emergencies. This is a unique position where you can make a difference to this diverse population of patients with complex medical needs. Pay rates range from $49.50 to $52.25 per hour based on employment status, shift, specific facility, site needs, licensure, and experience. *New grads are encouraged to apply - we'll train you in the exciting field of correctional healthcare!* Responsibilities As a Registered Nurse you will play a critical role in our continued mission to provide quality care that protects the rights, dignity, and health of our unique patient population. Duties and responsibilities may include and are not limited to: Performing initial health screenings and physical assessments of newly detained individuals. Monitoring and documenting patient health including vital signs, medical histories. Providing direct patient care, triaging, assessing patient needs, developing care plans, and coordinating services. Managing acute and chronic conditions such as asthma, hypertension, and diabetes. Responding to medical emergencies, providing first aid, and ensuring timely transport to appropriate healthcare facilities. Educating patients on preventative care, healthy lifestyle choices, and substance abuse prevention. Understanding and adhering to correctional facility security protocols, while working closely with correctional officers and other healthcare professionals, to ensure the safety and well-being of patients. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. Communication 1. Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. 2. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. 3. Communicates with community health related persons to coordinate the care plan. 4. As a mandatory reporter, reports failure to comply with the requirements of VA state, local, and federal law and ethics as required within 14 calendar days, using Dept. of Health forms. 5. Reports suspected abandonment, abuse, financial exploitation, or neglect of a person to the department of social and health services and the proper law enforcement agency. Reports must be submitted immediately when the reporting person has reasonable cause to believe that abandonment, abuse, financial exploitation, or neglect of a vulnerable adult has occurred. 6. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 7. Provides and maintains a safe environment for the patient. 8. Assists the patient and family/caregiver and other team members in providing continuity of care. 9. Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. 10. Attends interdisciplinary group meetings. Additional Duties 1. Participates in on-call duties as defined by the on-call policy. 2. Ensures that arrangements for equipment and other necessary items and services are available. 3. Supervises ancillary personnel and delegates responsibilities if required. 4. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes. 5. Fulfills the obligation of requested and/or accepted case assignments. 6. Actively participates in quality assessment performance improvement teams and activities. POSITION QUALIFICATIONS 1. Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice in the state of Washington. 3. Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing and minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Management experience not required. Responsible for supervising hospice aides may be applicable. 5. A Fred Hutch Cancer Center ARNP/PA-C is a certified physician assistant or advanced registered nurse practitioner who provides evaluation and medical management of Breast Oncology patients at the Fred Hutch Cancer Center. They follow patients during their chemotherapy and provide medical supportive care, as well as surveillance after completion of treatment. This position also provides opportunities for autonomous practice within the Women’s Wellness Follow-up Clinic The ARNP/PA-C assesses and manages basic health needs of patients, addresses treatment toxicities, performs procedures, monitors for cancer progression or recurrence, and consults with professional peers as needed. They work collaboratively with the treatment team to coordinate care of their patients. All clinical responsibilities are performed within the scope of practice, institutional privileges and under the supervision of an attending physician.
