All affiliated Home care aides employed by Nurse Next Door to provide in-home care services will be 18 years old (or if under 18 years of age be a high school graduate or enrolled in a vocational education program) and have sufficient communication and language skills to enable them to perform their duties and interact effectively with clients and other agency staff. Home care aides must meet all registration, criminal background checks and training requirements as specified by the State of California Department of Social Services Home Care Services Bureau and Nurse Next Door policies. Primarily responsible for providing professional nursing care to assigned patients. A nursing assistant is responsible for providing routine nursing care in accordance with established policies and procedures and as may be directed by the Charge Nurse, RN Supervisor, Director of Nurses or Administrator, to assure that the highest degree of quality resident care can be maintained at all times. QUALIFICATIONS • Currently enrolled in a school for nursing assistants or have a license as a certified nursing assistant. • A minimum of 6 months experience as a certified nursing assistant or clinical experience while enrolled in a nursing assistant certification class. • Must possess a sincere desire to serve the ill, aged and infirm. • Ability to read, write and speak the English language. • Possess the ability to deal tactfully with personnel, residents, family members, visitors, and the general public. • Willingness to work harmoniously with other personnel. • Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level they are currently functioning. • Must be in good general health and demonstrate emotional stability. • Must be able to relate to and work with ill, disabled, elderly, emotionally upset, and, at times, hostile people within the facility. Report missing ID bands to admission coordinator immediately. • Make beds, clean bedside and overbed tables. • Serve drinking water as required. Assure that water pitchers are cleaned, filled with fresh drinking water and within the resident’s reach. Pass water pitcher as per established policy and procedure. • Dress residents neatly and in their own clothing. Assist residents in dressing as necessary. • Assist in preparing residents for meals (taking to/from dining room, serving trays, placing bibs, assisting in feeding or cutting food, removal of trays, supervision in dining room, etc.). • Serve nourishment in accordance with established facility procedures. • Feed residents who cannot feed themselves. • Assure that resident’s food is accessible and self-help devices are available as needed. • Prepare residents for therapy, activities, physician visits, religious services, etc., to include transporting residents to/from such activities. • Keep incontinent residents clean and dry as possible at all times. Change bed linens, diapers and clothing as often as necessary. • Do not permit residents to smoke in bed. • Assure that the nurse’s call system is attached to the bed and within easy reach at all times when residents are bedfast. • Answer residents’ call lights promptly. • Check for food in residents’ room. Ensure that food is kept in a sealed container. • Report all complaints to the Charge Nurse. • Report any resident abuse immediately (ie., harsh/abusive language, unnecessary roughness, etc.) • Report any medication found in the resident’s room. Take to the Charge Nurse immediately. • Report all hazardous conditions and faulty equipment to the Charge Nurse immediately. • Report any bruise, skin tears, incidents or accidents to the Charge Nurse immediately. • Attend in-service educational programs, on the job training programs and meetings as directed. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. CLINICAL • Wash hands before and after caring for each resident. • Place residents in correct and comfortable position. Head of bed elevated at 30 for all residents on enteral feeding. • Assist residents with dental and mouth care. Clean dentures after each meal. Keep dentures clean and soaked with Efferdent after dinner. • Record resident’s food and nourishment intake as directed. • Perform after meal care as required – cleaning resident’s hands, face, clothing, dentures, etc. • Assist residents to/from bathroom. Offer bedfast residents bedpans and urinals. Clean and store such equipment after each use (document Intake and Output as directed). • Check and document bowel movements and character of stools every shift. • Bathe residents as assigned and in accordance with established facility procedures (encourage showers and other self-help measures/activities). • Comb and brush resident’s hair daily. Shampoo hair on shower days. • Clip and trim fingernails. • Shave male residents daily or as needed. • Keep hair under arms and face on female residents clean-shaven as necessary or instructed. Trim hair in nose and ears as needed. • Observe and report the presence of reddened areas on bony prominences. • Keep drainage bag off the floor when resident is in bed or in wheelchair. • Apply Vaseline or A & D ointment to dry skin in accordance with established policies. • Move and turn bedfast residents at least every two hours and record such information on CNA notes. • Observe disoriented and comatose residents and notify Charge Nurse immediately. • Apply postural support properly. Check with Charge Nurse for type of restraint to be used. • Apply hand roll or splint to residents with contractures as outlined within the facility’s established policies and procedures. • Follow infection control procedures in accordance with established facility policy when isolation techniques are necessary. • Chart required information every shift. • Complete Intake and Output documentation on all residents with orders before the end of each shift. • Other related duties as may become necessary or as directed by the Charge Nurse, RN Supervisor, Director of Nurses, or Administrator. CONSUMER SERVICE • Presents professional image to consumers through dress, behavior and speech. • Adheres to Company Standards for resolving consumer concerns. • Ensures that all consumer/resident rights are protected.

C. Scope of Practice of the PA: 1. Evaluates and treats patients with acute, chronic complaints and health maintenance concerns related to specialty. 2. Obtains complete histories and performs pertinent physical exams with assessment of normal and abnormal findings on new and return patients. 3. Performs or requests and evaluates diagnostic studies as indicated upon evaluation of the patient. Orders or furnishes medications under physician supervision, in accordance with the practice agreement, consistent with the PA’s educational preparation and training, and upon completion of a California Physician Assistant Board approved controlled substance course. a. All PA’s who are credentialed and privileged by April, 2021 who have not completed a controlled substance course must successfully complete the course by August 31, 2021 during which time patient specific approval of a medication order by a supervising physician is obtained, documented in the medical record, and is co-signed. Orders and collects specimens for routine laboratory tests, screening procedures and therapeutic procedures, including blood and blood products. 5. Performs designated procedures after demonstrated competency, according to written standardized procedures where applicable. 7. Obtains informed consent, as indicated. 8. Initiates arrangements for hospital admissions and discharges and completes appropriate paperwork. 9. As directed by the supervising physician, enrolls patients in investigational studies approved by the Investigational Review Board (IRB), and orders the necessary tests and medications. After appropriate training, assists the supervising physician in the operating room (OR). a. Acts as first or second assistant under the supervision of an approved supervising physician. b. Performs surgical procedures in the personal presence of the supervising physician. 11. Recognizes and considers age-specific needs of patients. 12. Effectively communicates and interacts with patients, families, staff and members of the community from diverse backgrounds. 13. Recognizes situations which require the immediate attention of a physician, and initiates life-saving procedures when necessary. 14. Facilitates the coordination of inpatient and outpatient care and services as needed. 15. Facilitates collaboration between providers and coordination of community resources. 16. Assess environmental control through surveillance of water supply systems as needed, air pressure relationships for high risk environmental monitoring. Conduct environmental rounds in all inpatient and outpatient care areas. Collect data on the incidence of selected device use in identified intensive care units. Participate in investigations of unusual hospital infection outbreaks utilizing the microbiology laboratory, consultation with Infectious Disease Division, county/state public health departments, and when necessary in consultation with the Center for Disease Control. Report epidemiologically significant findings to appropriate customers. Evaluate the effectiveness of the surveillance plan and modifies as necessary. Report all in-house patients with communicable disease to the county and/or state health department and maintain appropriate records. Compile and interpret surveillance reports to Infection Control committee, specialty areas, and executive medical committee on a regular basis. Collaborates with the CNO and directs and participates in the preparation and administration of nursing services, budgets and capital outlay requests; evaluates staffing, equipment and supply needs; prepares justifications and cost benefit analyses; reviews and recommends approval of expenditures; oversees the procurement of supplies and equipment for assigned areas. Satisfactory completion of a Certified Nursing Assistant training program. Licensure/Certification Requirements CNA certification required. American Heart Association Basic Life Support (BLS) certification required. Experience Requirements Previous six (6) months experience as a certified nursing assistant required or experience in the military medical service (US Navy hospital corpsman or US Army/Air Force healthcare specialist) position(s) performing similar duties will also qualify; however, those in like-minded military position must attain a CNA certification within 1 year of hire. Special Skills or Abilities Good organizational skills to handle a large volume of patient care activities. Emotional and physical stamina to work in a stressful work environment related to patients, staff and visitors.

Job Description Summary A Home care aide is an individual authorized to provide nonmedical services and assistance to an elderly or disabled client. Care Is usually provided at the person’s home, or facility. Nurse Next Door affiliated home care aides shall be selected on the basis of such factors as a sympathetic attitude towards the care of the sick, ability to carry out directions, maturity and ability to deal effectively with the demands of the job. All affiliated Home care aides employed by Nurse Next Door to provide in-home care services will be 18 years old (or if under 18 years of age be a high school graduate or enrolled in a vocational education program) and have sufficient communication and language skills to enable them to perform their duties and interact effectively with clients and other agency staff. Home care aides must meet all registration, criminal background checks and training requirements as specified by the State of California Department of Social Services Home Care Services Bureau and Nurse Next Door policies. Essential Job Functions/Responsibilities Responsibilities of the home care aide include, but are not limited to, the following- · Execute the client’s care plan as outlined by the Care Designer. · Help clients with their ADLs (Activities of Daily Living) that are within the scope of practice for a Registered Health Care Aide. · Assisting clients with personal hygiene such as skin, mouth, hair care, bathing and dressing. · Assisting clients in and out of bed, transfers, positioning and assisting with ambulation. · Assisting with prescribed exercises which clients and aides have been taught by appropriate health personnel. · Meal planning and preparation, including therapeutic diets, and assisting clients with eating. · Assisting clients to the bathroom or in using commodes, bedpans or urinals and incontinence care. · Performing household services including housekeeping, laundry, transportation, correspondence, making telephone calls, shopping for personal care items or groceries, and companionship. · Assisting clients with medications reminders which are ordinarily self-administered. · Reporting changes in the client's condition and needs to the administrator or care designer. · Completing records regarding services performed. · Driving to appointments or to run errands. · Accurately records all hours worked. · Remains alert throughout the shift in case client requires assistance, responds immediately to any such needs by providing the assistance required, or calling the Company for guidance. · Participating in all required entry-level and annual training requirements. · Maintaining an active home care aide registration with the State of Ca. Dept. of Social Services Home Care Services Bureau. · Other duties as assigned by the administrator or designee. Position Qualifications · At least 18 years of age. · Current registration with the State of California Home Care Aide Registry. We Provide paid training and help you with the registration process. · Completion of a criminal background check and disclosure of any criminal history or evidence of exemption. · Evidence of entry-level training or plan to complete paid training prior to client care and in accordance with CA regulations. · Evidence of tuberculosis testing and verification that the home care aide does not have active tuberculosis. · Ability to read and follow written instructions and document care given. · Self-directing with the ability to work with little direct supervision. · Empathy for the needs of the ill, injured, frail and the impaired. · Demonstrates tact, patience, and good personal hygiene. · Valid driver’s license and auto insurance and/or reliable transportation required. Physical Demands · Regularly required to sit, use hands or fingers, handle, or feel and talk or hear. · Required to reach with hands and arms. · Required to stand, walk, climb or balance, stoop, kneel, crouch, or crawl, and taste or smell. · Must be able to lift and or move up to 10 pounds, frequently lift and or move from 25 up to 50 pounds. · Specific vision abilities required by the job include close vision, distance vision, peripheral vision, depth perception and ability to adjust focus.
