JOB DESCRIPTION Summary The position works under the direction of, and in collaboration with the Acute Care/Emergency Department, Director of Nursing, and providers, to ensure quality patient care and compliance with regulatory standards for the area. The position will provide oversight to nursing care while leading, coaching, mentoring, and instructing clinical staff. This will include assistance in providing orientation and ongoing training and development. Stroke and Trauma Job Duties Assists in the monitoring of the ER and AC staff, identifying needs for improvements and work with the DON to provide training and education ensuring all staff are performing competently, safely and efficiently in trauma and stroke care. Participate in the development and planning of goals and objectives related to trauma care. Coordinate and participate in Trauma committee and complete chart review process through tertiary review. Collaborate with finance for monitoring of trauma and stroke patient financial reimbursement issues as directed by DON. Participate in the budget process: anticipate trends, future needs of the trauma program. Serves as a liaison to administration, representing the Trauma and Stroke Program on various hospital and community committees to enhance and foster a fiscally sound Trauma and Stroke Program. Serve as liaison for acute care and emergency department to monitor for educational requirements as needed for trauma and stroke designation. Supervise adherence to hospital policies and procedures and standards through observation, medical record review, staff feedbacks and other appropriate sources for trauma and stroke best practice. Interpret and implement policies and procedures; make recommendations for revisions; assist with updating policies and procedures in relation to trauma and stroke care. Maintains and/or coordinates trauma and stroke registry data. Attends Regional stroke and trauma coordinator meetings, fostering relationships within community. Coordinates stroke and trauma presentations and visits. Participate in case review. Develop and monitor the trauma/stroke PI program in collaboration with the trauma/stroke program assistant medical director. Communicate effectively with a wide variety of intra- and inter-facility staff and administration using both oral and written communication. Ensures timely primary, secondary and tertiary review of trauma charts. Coordinates Trauma and stroke PI process according to pre-determined criteria. Organize required quarterly trauma/stroke meetings and maintain meeting minutes per required documentation to meet designation criteria. Coordinate orientation, cross training, new equipment orientation, and skill development in relation to trauma and stroke care. Develop and implement strategies for communication, education and feedback for EMS systems in the catchment area. Direct community trauma/stroke education and prevention programs by developing, implementing and evaluating programs for targeted populations in the community related to injury prevention and other topics identified through needs assessment of the community. Monitor physician and nurse compliance with the educational requirements of the trauma/stroke program. Summary The RN Supervisor- Clinic and Care Coordination is responsible for providing day-to-day clinical leadership and operational oversight of care coordination services (Chronic Care Management, Transitional Care Management, and general case management), as well as overall clinic nursing operations. This position works in partnership with clinic management, providers, and nursing staff to ensure high-quality, patient-centered care, optimize clinical workflows, ensure regulatory compliance, and support care transitions and chronic disease management. This role is vital to ensuring staff are competent, confident, and operating within their scope of practice, while also maintaining a strong presence in provider relations, patient experience, and care continuity. The RN Supervisor functions as both a strategic and hands-on leader, balancing administrative responsibilities with direct patient care and mentorship when needed. Clinical Oversight & Supervision • Rotate between all clinic locations on a regular schedule • Provide daily clinical supervision and leadership to RNs, LPNs, Clinical Assistants (MAs), and care coordination staff. • Delegate and manage daily nursing assignments based on staff scope of practice, patient care needs, and coverage demands. • Supervise care coordination programs including CCM and TCM, ensuring accurate documentation, patient eligibility tracking, time logging, and billing workflows. • Serve as a clinical escalation resource for care coordination and complex patient cases. • Monitor patient care delivery, promoting evidence-based practices and quality improvement. Staff Development & Performance Management • Participate in recruitment, onboarding, performance evaluations, staff development, and corrective actions. • Identify training needs and facilitate or coordinate regular in-services and competency validation. • Ensure staff maintain current credentials and complete mandatory trainings. • Provide hands-on training and mentorship for both new and existing staff in lab, clinic, and care coordination workflows. Regulatory Compliance & Quality Assurance • Ensure compliance with CMS, MDH, CDC, OSHA, and other regulatory bodies related to clinical operations and documentation. • Oversee clinic infection prevention practices and proper storage/handling of medical supplies and vaccines. • Conduct internal audits and mock surveys to ensure clinical readiness and documentation quality. • Assist in development, implementation, and revision of policies, procedures, and workflows. Leadership & Program Implementation • Serve as a liaison between clinical support staff and medical providers to address issues, improve communication, and enhance collaboration. • Lead or participate in strategic initiatives and clinical committees (e.g., Pain Committee, Diabetes Advisory Committee). • Support implementation and evaluation of new care models, including the programs like chronic care management. • Assist in planning and resource allocation for new programs or population health initiatives. Patient Care & Community Engagement • Provide direct patient care when necessary, especially during staffing shortages or peak patient volume. • Support community health efforts, including wellness initiatives, chronic disease management, and prevention programs. • Engage in patient education, especially for chronic conditions such as diabetes, hypertension, COPD, and care transitions. Position Type/Expected Hours of Work Provide leadership and guidance to the department Monday through Friday from 8:30 a.m. to 5:00 p.m., with rotating Saturday mornings. Hours may vary earlier or later depending on workload. This role will rotate through clinic locations on a set schedule, working in different clinics as assigned. Additionally, availability outside of regular business hours may occasionally be required to address urgent questions or operational needs. Supervisory Responsibility This position directly supervises RN/LPN care coordinators, clinical assistants (MAs), and other clinical support staff across family medicine clinics.
