What You’ll Do: Provide nursing care and education to patients on anticoagulant therapy. Monitor and adjust anticoagulation plans under provider guidance. Perform INR testing and interpret results to ensure safe therapeutic levels. Educate patients on medication adherence, dietary interactions, and symptom monitoring. Communicate with providers regarding dosage adjustments and care plans. Document assessments, interventions, and patient interactions in the EMR.
