Visiting Nurse & Hospice of Litchfield County is currently looking for a Home Health Aide/Certified Nursing Assistant to join their team. The ideal candidate should possess a minimum of 1-2 years Home Health Aide experience working for a licensed and Medicare Certified Home Health or Hospice agency. Responsibilities include: making visits to patient homes to provide personal care activities including bathing, oral hygiene, feeding, dressing, and assisting the patient with ambulation and transfer activities. Must be flexible, reliable, and possess good organizational and communication skills. Per Diem/Part-time hours available serving the towns of: Salisbury, Sharon, Canaan & Norfolk.

Diagnose and develop individualized treatment plans based on the patient's mental health needs. Provide medication management and adjust treatment plans as necessary. Collaborate with interdisciplinary teams, including physicians, psychologists, and social workers, to ensure holistic patient care. Offer counseling and psychotherapy services to patients and their families. Educate patients and their families about mental health conditions, treatment options, and self-care strategies.
