Unit Summary This Oncology Intermediate Care Unit is a 16- bed critical care unit that consists of 2 private rooms and 7 semi-private rooms. OCU cares for adult acute care patients who have a wide variety of diagnoses and co-morbidities providing comprehensive care according to the scope of care defined by the hospital, Massachusetts Nurse Practice Act, American Nurses Association scope and Standards of Practice, and Quality Caring framework and professional practice model at Lowell General Hospital. Diagnoses include but are not limited to; chest pain, congestive heart failure, new onset A-fib, post cardiac catheterization patients, COPD, patients requiring high flow and bipap, GI bleed, and we specialize in oncology/hematology which includes administration of chemotherapy. The nurse:patient ratio is less than on the medical surgical floors, allowing patients to be more closely monitored. Job Summary: The Clinical Care Manager-Massachusetts is a community based registered nurse responsible for providing monitoring, follow-up and clinical care management to dually-eligible enrollees with complex medical, behavioral and social care needs. This position focuses on integrating health services and community resources to coordinate enrollee care for improve health outcomes and enhanced quality of life. Essential Functions: Engage with the enrollee in their homes and other community settings to establish an effective, complex care management relationship, while considering the cultural and linguistic needs of each member. Function as a liaison between healthcare providers, community resources, and enrollees to ensure seamless communication and care transitions. Manages patients with chest drainage devices. Manages patients with Intravenous Therapy. Uses Electronic Medical Record appropriately and accurately. Manages patients receiving epidural analgesia. Demonstrates knowledge and skill in administration and assessment of patient response to cardiac medications. Manages patients on telemetry monitoring. Responds appropriately in emergency/urgent situations. Provides care to post-operative patients. Cares for patients with orthopedic diagnoses. Other duties may be assigned. Graduate of an accredited School of Nursing. Current Registered Nurse (RN) Licensure in the Commonwealth of Massachusetts. Maintains BLS certification. Successful completion of Medical/Surgical IV Medication Module within 3 months of hire. Successful completion of Basic Arrhythmia exam, before hire. Bi-Pap Competency within 3 months of hire and then yearly for RNs caring for patients requiring Bi-Pap (MS1 RNs only) 72PP (36 Hours Per Week) Rotating Evenings, 7p-7a, Every Other Weekend & Rotating Holidays (Med Surg 2) *Sign-On Bonus Eligible! The Registered Nurse RN consults as needed with the physician and the office giving details about patient care. Performing accurate OASIS collection, ensuring the medication profile remains current, ensuring lab values have been reported to the physician timely, attending weekly case conference and monthly case manager meetings are key responsibilities of this role.
