Responsibilities Intake Coordinator- RN/ Social Work- Behavioral Health- Part Time Horizon Health is seeking a Clinical Assessment Coordinator for our unit at Johnson Memorial Hospital in Franklin, IN. The Clinical Assessment Coordinator, in conjunction with the unit’s Program Director may coordinate the referral development and pre-admission patient assessment process and provide clinical assessments for potential clients. Responsibilities: In coordination with the Program Director, develops, implements, coordinates, and evaluates clinical admission, referrals, and/or discharge processes. Conducts on and off site patient assessments, including appropriate test administration, interpretation and recommendations Responds to intake calls and completes admission paperwork Participates in the development of treatment plans in coordination with the treatment team Coordinates treatment with physicians and other health and social agencies Facilitates groups, family and individual sessions and completes all necessary documentation Coordinates and participates as a direct liaison with appropriate interested parties and physicians through the patient referral inquiry, admission, treatment, and discharge process Reviews newly admitted patient accounts for eligibility and authorization daily Maintains schedule for filing court paperwork and hearings Addresses issues in a timely manner including crisis and higher level of care referrals. Case management: communicates with personnel from outside facilities, participates in discharge planning, manages participant recertification. Establishes and maintains cooperative working relationships with other program coworkers, contract agencies and outside organizations. Obtains and manages outside medical records. Demonstrates clinical skills regarding application of Franciscan Senior Health and Wellness PACE policy and procedures, physical assessments, symptom management and participant relationships. Maintains compliance with nursing competencies. Demonstrates a working knowledge of nursing contribution to the PACE quality program. The registered professional clinical nurse (RN) is accountable and responsible for the provision and coordination of competent, evidence-based care along the care continuum, which includes assessment, diagnosis, outcome identification, planning, implementation, and evaluation. The clinical nurse participates in decision-making, teaching, mentoring, and collaboration with the multidisciplinary team to advance the patient plan of care, achieve outcomes and enhance operational efficiency. Job Summary : To assist in the delivery of health care and patient care management in a clinical capacity. Daily responsibilities of the position are based solely on the needs of the practice at any given time. Job Description Essential Responsibilities: 1. Provides directly and through delegation patient care for facility clients. Maintains knowledge of normal/abnormal parameters that identify clients at risk. Provides patient care in accordance to standards and guidelines. Responds to telephone calls and triages appropriately. Provides patient with reassurance, accurate teaching and direction. 2. Ensures efficient delivery of healthcare services throughout the duration of the patient visit. Ensures that medical records are completed, accurate and updated. Initiates patient contact and prepares patient for examination. Obtains vital signs, weight, age, temperature, blood pressure, pulse, respiratory, history and chief complaint. Observes patients with abnormal signs and symptoms, communicates their condition to the physician and documents the same in the medical record. Ensures patient understanding of physician instruction upon discharge. Documents patients medical record entries and documents charge information into electronic medical record for billing. 3. Assist with testing and treatment procedures under the Physicians supervision. Assists physician in applying splints and dressings. Performs and follows through in all indicated orders from physician. Assists with sample medications and sample medication logs. Arranges and administers any necessary patient testing or admission. Obtains appropriate referrals required. Retrieves test results as needed and notifies patient. Assists physician in preparing for minor surgeries and physicals. Obtains appropriate consents before all invasive procedures or immunizations are preformed. Distributes patient education materials as directed. Assists in patient instruction and ascertains patient understanding regarding treatment and medication usage. Performs laboratory tests and treatments as prescribed. Assists in maintaining laboratory log. 4. Ensures all patient care areas, equipment and materials are appropriately stocked and maintained. Maintains and organizes adequate supply of patient education materials. Orders and maintains required supplies and equipment for clinical care areas ensuring exam rooms are stocked appropriately. Keep exam rooms clean, free of waste and organized in accordance with cleaning protocol. Ensures that instruments are sterile and equipment is in working order. 5.
