Duties and Responsibilities: · Conducts home assessments (and virtual assessments, as needed) for prospective members and re-assessments of current members using the Uniform Assessment System (UAS); make enrollment and continued eligibility determinations by utilizing clinical expertise and critical thinking skills. As needed, conducts assessments in settings other than home as needed, e.g. skilled nursing facilities or virtually · Navigates multiple technology//digital platforms such as UAS software, electronic medical records, TruCare, etc., to conduct and document comprehensive assessments accurately and efficiently · Completes assessment of service needs per CMT and member's family//caregiver’s request to determine service needs or any necessary adjustments · Reviews previous nursing assessments//medical notes to determine necessary adjustments//updates in Care Plan and//or to assist with nursing home admission · Conducts in-person Care Management visits in support of person-centered service planning · Liaisons between the member and the CMT; assesses home environment and psychosocial status · Provides feedback to home care agencies and CMT regarding home care issues and contract nurses and aides performance · Provides grass roots, community-based training for frail population care including self-care techniques and prevention strategies · Ensures (prospective) members and significant others//responsible parties understand and are in agreement with enrollment in a managed long-term care plan · Additional duties as assigned Minimum Qualifications: · New York State RN license · Ability to travel around downstate New York which includes the 5 boroughs, Long Island, Rockland, and Westchester. · Prior work experience requiring technological proficiency including the ability to navigate multiple technology platforms and modalities · Prior work experience requiring intermediate Microsoft Word, Excel, and Outlook skills · Prior work experience requiring time management, critical//creative thinking, communication, and problem-solving skills · For PEDS positions only: 1 year of pediatric clinical field experience and//or experience with families and child serving systems, including child welfare and//or medically fragile//developmentally disabled populations Preferred Qualifications: · Work experience using electronic patient health information (PHI) database usage especially UAS · Previous field-based experience assessing, planning, and evaluating member's care by making home or facility visits for intake//reassessments or start of care (SOC) Community Home Health Agency (CHHA) visits · Work experience preferred in one or more of the following areas, geriatrics, home care, discharge planning, case management, and//or medical surgical nursing · Knowledge of health insurance, Medicaid, Medicare and MLTCP · Experience working with a frail adult or elderly population with the ability to determine appropriate care plans and services for frail population as well as negotiate initial service plans so that members and families are in agreement · Language preferences - Spanish, Russian, French, Creole, Mandarin, Cantonese · Demonstrated ability handling heavy caseloads · NY state driver’s license Hiring Range*: Greater New York City Area (NY, NJ, CT residents): $100,000 - $120,000 All Other Locations (within approved locations): $71,600 - $106,505 $10,000 Sign-On Bonus As a candidate for this position, your salary and related elements of compensation will be contingent upon your work experience, education, licenses and certifications, and any other factors Healthfirst deems pertinent to the hiring decision. Description The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Patient Care 1. Acts as a liaison with patient/families during the transitional period between the time the patient is discharged until admission visit. Nurse Practitioner – Ambulatory Surgical Center Job Summary: The Nurse Practitioner provides high-quality, patient-centered care throughout the perioperative process. Sign on Bonus UP TO $4,000 RN Manager Job Description: To function at a professional nursing level using both administrative leadership and clinical expertise in order to assure that comprehensive care and treatment is rendered to resident population, collaborates with the interdisciplinary team in the design, implementation and evaluation of various programs within the unit. Twenty-four hour responsibility for the continuity of nursing care and the management of the resident welfare. Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary Day to Day Supervision of Licensed and Non-Licensed staff, including counseling/disciplinary action, evaluations, one-to-one in service, suspension Maintain an ongoing quality assurance program for the nursing unit Develop methods for coordination of nursing services with other resident services to ensure the continuity of the residents total regimen of care Must possess the ability to make independent decisions when circumstances warrant such action. Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public. Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care. Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel. RN Manager Requirements/Qualifications: Must have a current NYS RN license. Graduation from an accredited school of nursing. MUST HAVE at least 2 years long term care experience 2 years of experience as a supervisor in a long-term care facility, or other related health care facility preferred Ability to read, write, and follow oral and written directions and work autonomously. Must be capable of maintaining regular attendance as requires. Related experience at a level necessary to accomplish this position. Must be able to adjust to a flexible schedule to include evenings, nights and weekend work hours. Serenity Hospice is looking to add a dedicated and compassionate LPN / Licensed Practical Nurse to maintain the comfort and optimize the quality of life of terminally ill patients and their families under the supervision of the Registered Nurse. This position will oversee the Bergen/Morris County area. Responsibilities: Provide direct care at the patient’s place of residence as indicated. Work collaboratively as a member of the Interdisciplinary Team to assure that the aims of continuity and comprehensiveness of care are accomplished. Understand Hospice philosophy, principles of death / dying. Compassionate communication with their patients and their families. Provides professional nursing care by utilizing all elements of nursing process. Collaborates with the patient and family to promote the physical well-being and address coping issues that may arise. Prepare documentation for the clinical record per Hospice standards in a timely manner. Participate in taking on-call responsibilities as necessary including weekends and evenings. Adhere to practice of confidentiality regarding patients, families, staff and the organization. Have observation skills and knowledge of when to alert doctors and other staff about changes in the patient’s condition. Reporting patient’s status regularly to RN Case Manager. Oversee Serenity Hospice Home Health Aides in the Facility. Assesses for infection control for protection of the patient as well as family/caregiver and staff safety, as well as reporting any suspicious infection to the infection control nurse and or hospice supervisors. Will be committed to ensuring that Serenity Hospice Care provides care and services of the highest quality and safety, and will continually strive to improve performance. Requirements: Licensed Practical Nurse in the state of New Jersey. Hospice experience preferred. Strong communication and interpersonal skills required. Computer proficiency.
