Summary of Position: The Perioperative Surgical Home (PSH) RN is responsible for the delivery of safe, effective, and quality patient-family centered care in the pre-admission testing unit. The Perioperative Surgical Home RN is responsible for providing professional nursing care to patients in the Perioperative Surgical Home Department, utilizing established policies and procedures, ensuring safe and effective care. This position is responsible for accurately conducting and documenting patient assessments, medical histories, patient teaching, and medication reconciliation. The PSH RN cares for a varied caseload of surgical patients, making advanced assessments of the patient’s physical and psychosocial needs. The PSH RN systematically and continuously collects and assesses data in collaboration with the multidisciplinary team to provide therapeutic care for the patient and/or family. Services are provided based on the ages of the population served; 1 year through geriatric - 65 years or older and in collaboration with the multidisciplinary care team. Demonstrates the knowledge and skills necessary to identify and meet the individualized needs of all patients served in the Perioperative Surgical Home Department.

Recognizes and accurately reports relevant resident information to licensed staff. • Reports on all significant changes in resident condition or motivational level. • Notifies charge nurse of feeding problems, reassessment needs. • Notifies PT and charge nurse of any mobility problems, referral or reassessment needs. 6. Utilizes time, equipment, and supplies in an efficient & safe manner. 7. Completes all assigned duties during designated shift. Prioritizes resident care activities based on care needs. Recognizes need for purpose vs. task when caring for residents. 8. Provides personal care assistance to residents, encouraging self care and independence but offering and providing help as needed: • Personal hygiene – bathing, bathroom/bedpan, oral care, AM and PMcare • Positioning • Dressing • Ambulation & transfers • Eating • Transport 9. Under direction of the therapist and charge nurse, performs routine physical therapy treatments • Range of Motion • Strengthening Exercises • Ensures placement of restorative devices 10. Performs basic procedures & care to including: • Application of simple dressings within CNA scope of practice • Specimen collection • Reinforces team management of bowel/bladder program • Assists residents with nutritional intake. 11. Communicates appropriate information regarding resident safety to charge RN 12. Follows all appropriate infection prevention measures in place in the SNF at all times. 13. Ensures that all aspects of resident care are documented. Documentation is complete, accurate and timely. 14. Supplies and Equipment • Operates equipment and ensures proper functioning of equipment • Follows procedure when equipment malfunctions. • Ensures use of supplies are accurately documented 15. Demonstrates knowledge and skills necessary to provide care appropriate to the age of the residents served. 16. Demonstrates a working knowledge of regulatory and licensing standards and ensures that practice is compliant with standards 17. Adheres to all hospital and departmental policies regarding attendance and punctuality. 18. Responds to the needs of the department by performing other duties, as necessary.
