Leads the execution of the strategic vision for care management across the organization, enhancing the consumer experience and deploying Care Management resources effectively. Develops strategies to reduce length of stay, readmissions, and clinical denials while simplifying daily workflows and supporting improved governance and accountability structures. Implements policies to decrease variability in care management practices and embed evidence-based care transition practices. Advises and supports the standardization of care management roles, responsibilities, job descriptions, and accountability metrics across the enterprise. Sets care management benchmarks and tracks performance across markets, providing data analysis and monitoring action plans when benchmarks are not met. Provides guidance and actively participates with facility staff to identify, implement, or enhance care management programs consistent with corporate and facility strategies. Determines proactive strategies and methods necessary for a high-performance culture related to care management and social work. Disseminates information accurately and timely with multiple audiences and leads regularly scheduled communication sessions with facility care management leadership. Identifies process improvement opportunities and develops, implements, monitors, and revises action plans. Collaborates with leaders from Utilization Review, Population Health, and Post-Acute, as well as other members of the clinical team, to ensure care coordination across all levels of care. Analyzes current and future regulatory/accreditation requirements, practice trends, and legislative issues to forecast potential impact on care management. Ensures the incorporation of best evidence into care management policies and practices systemwide and establishes mechanisms for sharing best practices across the system. Bay Area Direct Client Care has an immediate openings for experienced LPNs. We currently are offering Night shift position that can either be full time or part time to include Thur, Fri , Sat 7p-7am or Fri, Sat, Sun. This can be every week or every other week. Current shifts available at our Ruskin group home with current 5 patients with a Home health Aid present for support. These are medically complex individuals with Trach, feeding tube and ventilator, will train. Requirements 1year LPN experience Current CPR card Pass Local and Level 2 screen Lift minimum 50 lbs Staff receive Paid in 2 business days from shift Direct Deposit Medical Insurance eligibility Bay Area Direct Client Care has an immediate opening for an experienced home health aide/ CNA in our medical group home 7pm- 7am. Starting rate $20/hr (Training is at training rate $15/hr, first solo shift is $20). The schedule rotates from week to week to accomodate every other weekend. Week one only work Thursday night 7pm- 7am. Week 2 work Mon, Tues, Wed, Fri, Sat Sun 7pm-7am and back to week 1 next week. Pay period runs from Wed- Tues so will have one check for 36 hrs next check 48 hours weekly pay. NO SLEEPING IS PERMITTED. Currently 5 patient group home with Nurse at all times. Under general supervision, following established policies, procedures, and professional guidelines, delivers nursing care to patients.
