Case Manager

The Case Manager coordinates with the admission staff and clinical staff to facilitate the meeting of patient's treatment needs as well as management of utilization review activities for the inpatient programs in accordance with the Joint Commission, Federal and State regulations, Oceans' mission, policies and procedures and Performance Improvement standards. Acts as patient liaison for physician, clinical staff and family member's activities. Works in coordination with the Performance Improvement Coordinator to conducts closed and concurrent reviews of medical records. Interacts with members of the medical/clinical team to provide a flow of communication, ensure criteria for admission and continued stay are met and that the medical record documentation supports the level and intensity of service rendered as well as facilitating timely discharge planning. Works with the primary therapist and nursing staff to facilitate treatment planning documentation, family contact, scheduling appointments, absentees from programming and access to community resources. Essential Functions: Functions as patient liaison in the role of case manager to coordinate patient treatments with the physician, therapist and nursing staff. Compiles psychosocial data and scribes on form; completes integration of assessment and presents to multi-disciplinary treatment team for review and approval; Schedules appointments, follows up on absentees and completes continuing care plans. Attends treatment team meetings and communicates case management activities to the other team members. Identifies and reports appropriate use under-use, overuse and inefficient use of services and resources to ensure high quality patient care is provided in the least restrictive environment and in a cost-effective manner. Conducts review, when needed, of records in a timely manner to (1) determine appropriateness and clinical necessity of admissions, continued stay, and/or rehabilitation, and discharge; (2) determine timeliness of assessments and evaluations; i.e., H&Ps, psychiatric evaluations, CIA formulation and discharge summaries; and (3) identify any under-, over- and/or inefficient use of services or resource. Notifies appropriate staff members of any deficiencies noted so corrective actions can be taken in a timely manner; submits monthly report to PI Coordinator of findings and actions recommended to correct identified problems. Coordinates flow of communication between physician/staff and third-party payers concerning reimbursement requisites. Attends status meetings to communicate third party payer status, update attendance calendar and communicate individual patient needs to clinical and admission staff. Shares pertinent data. Initiates and completes appeals process for reimbursement denials; notifies inpatients of denials received; notifies physicians/staff/patients of reimbursement issues. Upon notification by business office that potential exists to be included on a new managed care contract, makes contact with the managed care company and coordinates communications between administrator and the managed care company to obtain contractual arrangements. Maintains coordination of information requests from third party payers for annual renewal or update of existing contracts. Communicates to staff status of new/existing contracts. Performs additional case management duties as required and coordinates flow of communication amend staff involved in the patient's care; completes paperwork for judicial commitments and state bed packets. Completes referral process and necessary paperwork for all other levels of care and make follow-up appointments; including follow-up letters needed by the patient; completes all medication assistance forms and assists patients and families with follow-up care items. Conducts special retrospective studies/audits when need is determined by M&PS and /or other committee structure. Accurately does discharge reviews on 100% of all discharges per month; accurately inputs discharge review information into computer database; accurately send records for appeal in timely manner; accurately completes all initial CQI+ forms and questionnaires in a timely manner. Performs other duties and projects as assigned.

The Registered Nurse maintains standards of professional nursing practice in accordance with facility policy and procedures, other external governing and credentialing bodies, performance improvement standards, and psychiatric nursing standards. The Registered Nurse assesses the patient utilizing the nursing process to formulate an individualized plan of care for each patient, implements interventions, and evaluates outcomes consistent with policy and appropriate to the current client’s condition, which promote, maintain, and restore physical and mental health, and prevent illness. The Registered Nurse interprets age/disability specific information in response to client population served. The Registered Nurse provides for the safe delivery of medication in accordance with the policies and procedures, physicians orders and/or current practice. The Registered Nurse assumes responsibility for professional development, continuing education and maintaining licensure. Essential Functions: Utilizes the nursing process: assessment, planning, intervention, implementation, and evaluation and formulating nursing interventions directed towards development of an individualized plan of care and treatment modalities. Identifies risk levels, precautions, and expected outcomes individualized to the client to include discharge planning; prescribes interventions to attain expected outcomes; evaluates the effectiveness of interventions in relation to expected outcomes; conducts ongoing assessment of clients from admission to discharge; initiates medical/psychiatric crisis intervention to stabilize and ensure the well-being of the patient's conditions. Demonstrates knowledge of psychiatric disorders and appropriate nursing interventions; implements nursing interventions identified in the plan of care that is individualized to the patient needs. Applies appropriate interventions to progress the patient towards wellness and addresses maintenance of wellness. Administers medications as prescribed safely; provides education on prescribed medications; recognizes signs and symptoms of adverse drug reactions. Demonstrates professionalism, accountability, and responsibility for decision making, quality nursing care, and knowledge base of psychiatric nursing care. Reviews, revises, and updates medical records within facility guidelines and timeframes. Transcribes and implements physician's orders according to facility policy; documents care, progress, response to intervention in medical record in an accurate, thorough and concise manner. Follows up on client care responsibilities, communicates incomplete assignments, and current client needs to staff to assure continuity of care. Diagnose and treat common childhood illnesses. Provide anticipatory guidance regarding common child health concerns.• Provide childhood immunizations. Perform school and sports physicals. Perform therapeutic procedures and prescribe pediatric treatment and medications based on assessment results and standards of care within the scope of practice of a pediatric nurse practitioner as defined and approved on the provider privileges list. Privileges scope of care and standards of practice guidelines are as defined by the National Association of Pediatric Nurse Practitioners (NAPNAP). Examines, diagnoses, and treats acute episodic and chronic diseases, minor traumas and behavioral/psychological problems of newborns, infants, children, and adolescents up to age 23 years. • Identifies and manages specific pediatric illnesses. Prepares and reviews case histories and clinical records of newborns, infants, children, and adolescents up to age 23 years. Note, if the applicant is a FNP, a small percentage of adults will be assigned to maintain practice currency. Secures a health and developmental history from the patient or parent; records findings and makes critical evaluation. Examines patients, determines need for radiological examinations Laboratory tests, and interprets examinations findings and test results. Also, performs or requests special screening and developmental tests and interprets the results. Provides notification to patients of results and adjusts the treatment plans as clinically warranted. • Evaluates total health care needs of patients and develops a plan to meet these needs. Teaches patients and family member’s different ways to maintain or improve their health status. • Provides healthcare and preventative services to pediatric patients and their family members, including age specific anticipatory guidance in growth and development, nutrition, common illnesses, safety, and discipline. Evaluates clinical practice and collaborates with health care team members as the Primary Care Manager to improve patient delivery for an empanelment of pediatric patients. Makes appropriate referrals to general pediatrician, family practice physician, and/or specialty physician as warranted. Qualifications Minimum Qualifications: • Degree: Master's degree in nursing. • Education: Graduate from an academic program in the nursing specialty accredited by a recognized nursing accrediting agency (ACEN or CCNE). • Certification: National certification as a Pediatric Nurse Practitioner (PNP) or Family Nurse Practitioner (FNP) • Experience: Minimum of 24 months of full-time, dedicated pediatric patient care experience as a Nurse Practitioner within the last 5 years, in a credentialed primary care setting. This experience must be in a role specifically focused on pediatric patients. • Licensure: Current, full, active, and unrestricted Registered Nurse (RN) license and Nurse Practitioner (NP) Licenses • Controlled Substances: Must have and maintain an active, valid, current, and unrestricted State Controlled Substance License and be eligible to obtain a DEA license. * Life Support Certification: Possess current ARC or AHA BLS Healthcare Provider certification. * Security: Must possess ability to pass a Government background check/security clearance.
