The Opportunity: Join Fairbanks Memorial Hospital (FMH) as a CRNA and make a direct impact on the Interior Alaska community. This rare three-week-on, two-week-off schedule gives you 20 weeks each year to explore Alaska’s outdoors, such as hiking, fishing, hunting, skiing, snowmobiling, and northern lights viewing. Experience a bread-and-butter caseload including general surgery, orthopedics, OB/GYN, ENT, podiatry, healthy peds, and GI procedures, with CRNAs managing cases with physician oversight. While high-acuity cases are less frequent, the work is dynamic and engaging, keeping your skills sharp as part of a rural health system, without the constant intensity of a trauma-heavy environment. Shifts are 12 hours, but many days end earlier, offering even more flexibility and ensuring a balanced schedule with predictable downtime. Compensation : Salary DOE between $315K-$378K | $40K sign-on bonus | $15K annual retention | $5K performance incentive | $5K CME allowance Patients: 4-5 ORs daily with one additional out-of-OR assignment (OB or GI Procedures). In collaboration with the attending anesthesiologist, the CRNA will demonstrate proficiency in the management of the induction, maintenance, and emergence of anesthesia. Recognize abnormal patient responses to anesthesia or to any adjunctive medication or form of therapy. Initiate corrective measures. Determine the need for additional laboratory tests and provide resuscitative care as necessary. Evaluate and manage the patient in the immediate post-operative period. 9. Administers IV fluids relative to the patient’s medical history and surgical procedure. Administer blood products after obtaining a hematocrit and then consultation with the surgeon and/or anesthesiologist. 10. Documents the patient status in the anesthesia record appropriately. 11. Performs all required safety and performance checks on anesthesia-related equipment and verify the appropriate supplies before initiating care. Procure and assemble equipment and supplies as indicated if anesthesia tech is not available. 12. Reviews appropriate documentation for informed consent and identify appropriate surgical site during SBAR with circulating RN and holding room RN. Participates in surgical pause in the OR with entire surgical team in accordance with departmental and regulatory compliance standards. 13. Communicates appropriately with all individuals involved in the peri-operative and post-operative care of the patient. 14. In a life-threatening emergency, functions responsibly by providing anesthesia care in the absence of an anesthesiologist. 15. Assists with the teaching and evaluation of newly-hired CRNAs, GRNAs, SRNAs in the clinical area. 16. Description Under the supervision of an Attending Anesthesiologist, this employee administers, monitors and manages patient’s care before and after anesthesia. Relies on experience and judgment to plan and accomplish goals. Performs a variety of tasks. Reports to Attending Anesthesiologists. A certain degree of creativity and latitude is required. Nurse Anesthetists will rotate responsibility for daily scheduling of case coverage. Responsibilities In conjunction with attending anesthesiologist, Nurse Anesthetist will perform a thorough and complete pre-anesthesia assessment. Nurse Anesthetist is responsible for obtaining reviewing relevant tests and discussing findings with the Attending. The Nurse Anesthetist shall obtain or verify that an informed consent has been obtained for the planned anesthetic intervention from the patient or legal guardian. Discuss anesthetic options and risks with the patient and/or legal guardian after discussion with Attending Anesthesiologist. Nurse Anesthetists must document in the patient’s medical record that informed consent was obtained. The Nurse Anesthetist, in conjunction with the Anesthesiologist, must formulate a patient-specific plan for anesthesia care. The plan is based upon comprehensive patient assessment, problem analysis, anticipated surgical or therapeutic procedure, patient and surgeon preferences and current anesthesia principles. The Nurse Anesthetist shall induce anesthesia under the supervision of an Attending Anesthesiologist and maintain anesthesia at required levels. The Nurse Anesthetist shall continuously assess the patient’s response to the anesthetic and/or surgical intervention and must immediately notify the Attending Anesthesiologist of significant changes in the patient’s condition and intervene as required, under the supervision of an Attending Anesthesiologist to maintain the patient in a satisfactory physiologic condition. The Nurse Anesthetist must monitor the patient’s physiologic condition as appropriate for the type of anesthesia and specific patient needs: A. Monitor ventilation continuously. Verify intubation of the trachea by auscultation, chest excursion, and confirmation of carbon dioxide in the expired gas. Continuously monitor end-tidal carbon dioxide during controlled or assisted ventilation. Use spirometry and ventilatory pressure monitors. B. Monitor Oxygenation continuously by clinical observation, pulse oximetry, and if indicated, arterial blood gas analysis. C. Monitor cardiovascular status continuously via electrocardiogram and heart sounds. Record blood pressure and heart rate. D. Monitor body temperature continuously on all pediatric patients receiving general anesthesia and when indicated, on all other patients. E. Monitor neuromuscular function and status when neuromuscular blocking agents are administered. F. Monitor and assess the patient positioning and protective measures. The omission of any monitoring standards shall be documented and the reason stated on the patient’s anesthesia record. The Nurse Anesthetist shall be in constant attendance of the patient until another qualified health care provider has accepted the responsibility for care. 6. The Nurse Anesthetist is responsible for maintaining complete, accurate, and timely documentation of pertinent information on the patient’s medical record. Must document all anesthetic interventions, and patient responses. 7. The Nurse Anesthetist is responsible for transferring the responsibility for care of the patient to other qualified providers in a manner that assures continuity of care and patient safety. 8. The Nurse Anesthetist must adhere to appropriate safety precautions, as established within the institution, to minimize the risks of fire, explosion, electrical shock and equipment malfunction. Document on the patient’s medical record that the anesthesia machine and equipment were checked. Prior to use, the Nurse Anesthetist shall inspect the anesthesia machine and monitors according to established guidelines. The Nurse Anesthetist shall check readiness, availability, cleanliness, and working condition of all equipment to be utilized in the administration of the anesthesia care. When the patient is ventilated by an automatic mechanical ventilator, monitor the integrity of the breathing system with a device capable of detecting a disconnection by emitting an audible alarm. Monitor oxygen concentration continuously with an oxygen supply failure alarm system. 9. Precautions shall be taken to minimize the risk of infection to the patient, the Nurse Anesthetist, and other healthcare providers. 10. The Nurse Anesthetist shall participate in the ongoing review and evaluation of the quality and appropriateness of the anesthesia care as a member of the Department of Anesthesiology. Evaluation shall be performed based upon appropriate outcome criteria and reviewed on an ongoing basis. The Nurse Anesthetist shall participate in a continual process of self-evaluation and strive to incorporate new techniques and knowledge into the practice. 11. Responsibilities Responsible To Designated Attending Anesthesiologists/ Vice President, Nursing and Chief Certified Registered Nurse Anesthetist for performance appraisals and scheduling Responsible For The performance of pre-anesthesia assessment, induction and maintenance of and emergence from anesthesia, monitoring of the patient's physiological condition and performance of clinical intervention as necessary, under the supervision of an Attending Anesthesiologist. Principal Duties and Responsibilities Clinical Practice 1. In conjunction with Attending Anesthesiologist, performs a thorough and completed pre-anesthesia assessment. Responsible for obtaining and reviewing relevant tests and discussing findings with the Attending. 2. Obtains and/or verifies that an informed consent has been obtained for the planned anesthetic intervention from the patient or legal guardian. Discusses anesthetic options and risks with the patient and/or legal guardian after discussion with Attending Anesthesiologist. Documents in the patients' medical record that informed consent was obtained. 3. Formulates a patient-specific plan for anesthesia care in conjunction with the Anesthesiologist. The plan is based upon comprehensive patient assessment, problem analysis, anticipated surgical or therapeutic procedure, patient and surgeon preferences, and current anesthesia principles. 4. Induces anesthesia under the supervision of an Attending Anesthesiologist and maintains anesthesia at required levels. Assesses continuously the patient's response to the anesthetic and/or surgical intervention and must immediately notify the Attending Anesthesiologist of significant changes in the patient's condition and intervenes as required, under the supervision of an Attending Anesthesiologist to maintain the patient in a satisfactory physiologic condition. 5. Monitors the patient's physiologic condition as appropriate for the type of anesthesia and specific patient needs. * Monitors ventilation continuously. Verifies intubation of the trachea by auscultation, chest excursion, and confirmation of carbon dioxide in the expired gas. Continuously monitors end-tidal carbon dioxide during controlled or assisted ventilation. Uses spirometry and ventilatory pressure monitors. * Monitors oxygenation continuously by clinical observation, pulse oximetry, and if indicated, arterial blood gas analysis. * Monitors cardiovascular status continuously via electrocardiogram and when appropriate, heart sounds. * Records blood pressure and heart rate. * Monitors body temperature continuously on all pediatric patients receiving general anesthesia and when indicated, on all other patients. * Monitors neuromuscular function and status when neuromuscular blocking agents are administered. * Monitors and assesses the patient positioning and protective measures. The omission of any monitoring standards shall be documented and the reason stated on the patients anesthesia record. The CRNA shall be in constant attendance of the patient until another qualified health care provider has accepted the responsibility for care. 6. Maintains complete, accurate, and timely documentation of pertinent information on the patients' medical record. Must document all anesthetic interventions and patient responses. 7. Transfers the responsibility for care of the patient to other qualified providers in a manner that assures continuity of care and patient safety. 8. Adheres to appropriate safety precautions, as established within the institution, to minimize the risk of fire, explosion, electrical shock and equipment malfunction. Documents on the patients' medical record that the anesthesia machine and equipment were checked. 9. Inspects the anesthesia machine prior to use and monitors according to established guidelines. Checks the readiness, availability, cleanliness, and working condition of all equipment to be utilized in the administration of the anesthesia care. When the patient is ventilated by an automatic mechanical ventilator, monitors the integrity of the breathing system with a device capable of detecting a disconnection by emitting an audible alarm. Monitors oxygen concentration continuously with an oxygen supply failure alarm system. 10. Offers psychological support while explaining the anesthetic risk/benefit and obtaining an informed consent for all planned and potential forms of anesthesia. 5% Selects appropriate equipment and techniques for use in the accomplishment of goals outlined in the anesthesia care plan/management plan as evidenced by: Evaluation of the functional status of anesthesia equipment (machine, suction, laryngoscope, monitors, etc.) Assures presence and functioning of emergency equipment prior to initiation of any anesthetic technique. Calculation of drug doses; preparation and appropriate labeling of syringes. Appropriate selection of airway management equipment. Appropriate application and selection of monitoring modalities. 5% Supervises Student Registered Nurse Anesthetists (SRNAs) in the delivery of anesthesia care. Participates in education program for SRNAs. 2% Appropriately and effectively administers pharmacological agents. Administers drugs based on individual patient need, dosage and interaction. Follows hospital policy on controlled substances security and sign out. Evaluates patient response to pharmacological agents. Effectively treats any adverse drug reaction. 2% Terminates the anesthetic and emerges the patient from anesthesia. Prepares the patient for emergence. Demonstrates knowledge of extubation criteria. Extubates patient and evaluates ventilatory status. Determines patient stability prior to transport to PACU. Leaves the OR in a state of readiness prior to leaving for PACU. Transports patient safely to PACU. Gives a thorough report to the PACU nurse. 2% Evaluates the attainment of anesthesia care goals. Evaluates response to anesthesia care (immediate or delayed). Evaluates and documents patient status on admission to PACU. Recognizes current or possible problems/complications/side effects. Completes narcotic record appropriately. Completes and documents post-anesthesia rounds. 2% Assists with starting difficult intravenous lines, inserting difficult arterial lines, obtaining difficult arterial blood gases, performing emergency endotracheal intubation and cardiopulmonary resuscitation throughout the hospital. Pass the national certification examination within 24 months after graduation. License/Certification Required: Current New York State Registered Nurse's License. Discusses the anesthetics, drugs and techniques to be used or any other situation relevant to the anesthesia with the anesthesiologist, surgeon and/or obstetrician when indicated. - Helps position patient properly on operating table; applies, attaches and/or inserts devices for monitoring vital signs and establishes baseline vital signs. - Performs technical procedures: veni-puncture, arterial puncture, laryngoscopy, oral or nasal intubation (before or after induction of anesthesia as indicated to insure airway maintenance. - Induces and maintains anesthesia/analgesia at a required safe level and monitors support life junctions during the administration of anesthesia. Detects and manages untoward reactions to anesthesia agents and other drugs used during surgery.
