Summary

Under physician supervision, performs duties in a nursing capacity with a focus on preventive care and patient wellness. Responsible for encouraging and completing Annual Wellness Visits (AWV) for eligible patients to include comprehensive health screenings, care plan development, and accurate coding to support proactive health maintenance and chronic condition management.

 

1. *Commits to the KRMC mission, vision, values and goals and consistently demonstrates our core values.

2. *Conducts patient assessments to support preventive care planning and mitigate risk of future complications.

3. *Completes Annual Wellness Visits (AWVs) for eligible patients.

4. *Maintains accurate, timely, and objective documentation of all care team communications and patient interactions within the Electronic Health Record (EHR).

5. *Collects subjective and objective data and contributes to the assessment of the patient.

6. *Collaborates with the interdisciplinary care team to ensure seamless care delivery and documents updates in the patient's record.

7. *Collects and documents comprehensive patient health history, including medical, family, surgical, and social history, in the electronic health record (EHR) in accordance with facility policies.

8. *Obtains and documents a thorough and accurate medication and allergy history.

9. *Monitors and accurately records patient vital signs, including blood pressure, pulse, temperature, and respiration.

10. *Conducts comprehensive patient assessments that include screening for Social Determinants of Health (SDOH) to identify social, economic, and environmental factors affecting health outcomes.

11. *Conducts comprehensive nursing assessments, including evaluations of depression, safety risks, activities of daily living (ADLs), and cognitive function.

12. *Initiates and facilitates timely, sensitive, and culturally appropriate conversations with patients and families regarding Advance Care Planning (ACP), including goals of care, values, and preferences for future medical treatments.

13. *Conducts comprehensive health maintenance reviews and leverages electronic health record (EHR) data to drive proactive patient care.

14. *Captures and documents appropriate HCC (Hierarchical Condition Category) diagnoses.

15. *Educates patients on the purpose and benefits of the Annual Wellness Visit and promotes understanding of preventive care measures.

16. *Ensures accurate and timely documentation in the medical record in accordance with Medicare and organizational requirements.

17. Interacts professionally with patients/families and includes them in relevant aspects of the wellness planning process.

18. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in preventive care and wellness initiatives.

19. Participates in meetings, committees, and department projects as assigned.

20. Performs other related projects and duties as assigned.

 

EDUCATION AND EXPERIENCE:

Graduation from an accredited professional nursing program required. Current Registered Nurse licensure from the State of Nebraska or approved compact state of residence as defined by the Nebraska Nurse Practice Act required.

 

OTHER CREDENTIALS / CERTIFICATIONS
