HOW YOU'LL WORK: You'll perform personal care including but not limited to: bathing, grooming, skin care, toileting, and oral hygiene. You will effectively communicate and document any changes in patient conditions to the registered nurse or supervisor and maintain positive relationships with your patient and team. You'll be responsible for all practices and duties within the home health aide scope of practice as outlined by the state for its HHA personnel. MAJOR AREAS OF RESPONSIBILITY: Patient Care: Assist patients as needed with lifting and transfers. Communication: Maintain good relationships with the patient, the patient’s family, and staff. Reports any changes to the patient’s medical and living conditions to the supervisor or the case manager. Minimize Patient Risk: Ensure the living environment is healthy and safe. Policies: Completes all documentation following agency protocol and Medicare/Federal guidelines. Operations: Attend in-service training and mandatory agency meetings. Completion of skills competency test with 80% or higher score. Participate in in-service training and continuing education as required. STNA Qualifications: Valid STNA certification in the state of employment High school diploma or GED preferred. Prior experience in long-term care or assisted living is a plus but not required. Ability to follow directions and communicate effectively with residents and staff. Compassionate, dependable, and patient-focused. Physical ability to lift, move, and assist residents and perform job-related tasks. Current CPR certification (or willing to obtain upon hire). STNA Physical Requirements: Ability to lift/move up to 50 lbs frequently. Standing, walking, bending, and kneeling throughout the shift. Ability to respond to emergency situations quickly.
