Qualifications: Active RN license in the state of employment Associate or Bachelor’s degree in Nursing (ADN or BSN) Current CPR/BLS certification (required) At least 1 year of nursing experience, preferably in long-term care or geriatric settings Strong clinical assessment and critical thinking skills Excellent communication, leadership, and organizational abilities Proficient in electronic medical records (EMR/EHR)
Position Summary: The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a long-term care facility under the supervision of a Registered Nurse or Physician. The LPN is responsible for delivering safe, compassionate, and effective nursing care while supporting residents’ well-being and maintaining compliance with regulatory standards. Key Responsibilities: Provide direct nursing care to residents according to care plans and physicians’ orders. Administer medications and treatments accurately and in a timely manner. Monitor and document residents’ health status, reporting any significant changes to the RN or physician. Assist with resident assessments and the development of individualized care plans. Supervise and direct CNAs and other nursing assistants in delivering daily care. Ensure all documentation is complete, accurate, and in compliance with facility policies and state/federal regulations. Respond to emergencies and follow proper protocols. Support residents and families with health education and emotional support. Maintain compliance with infection control, safety, and HIPAA standards. Participate in interdisciplinary team meetings, audits, and in-service training. Qualifications: Valid LPN license in the state of employment Graduate of an accredited Licensed Practical Nursing program CPR/BLS certification (required or able to obtain upon hire) At least 1 year of nursing experience (long-term care preferred, but not required) Ability to communicate effectively and compassionately with residents, families, and team members Familiarity with electronic medical records (EMR) systems preferred Strong critical thinking, organizational, and time-management skills
Position Summary: The STNA provides essential hands-on care and daily support to residents in a long-term care facility. Working under the supervision of licensed nurses, the STNA assists residents with activities of daily living (ADLs), helps maintain a safe and clean environment, and promotes resident dignity and quality of life. Key Responsibilities: Assist residents with ADLs such as bathing, dressing, grooming, toileting, and feeding. Take and record vital signs (temperature, pulse, respiration, blood pressure) as directed. Assist residents with mobility, positioning, transfers, and use of assistive devices. Observe and report changes in residents’ physical or emotional condition to nursing staff. Answer call lights promptly and provide needed assistance. Help residents with meals, including tray setup and feeding assistance. Maintain accurate and timely documentation of care provided. Keep resident rooms clean, organized, and safe. Provide respectful, compassionate care in accordance with resident care plans. Follow facility policies, state regulations, and infection control protocols. Participate in in-service training and continuing education as required. Qualifications: Valid STNA certification in the state of employment High school diploma or GED preferred. Prior experience in long-term care or assisted living is a plus but not required. Ability to follow directions and communicate effectively with residents and staff. Compassionate, dependable, and patient-focused. Physical ability to lift, move, and assist residents and perform job-related tasks. Current CPR certification (or willing to obtain upon hire).
