The STNA works in accordance with facility policies and procedures and reports resident needs and concerns to a licensed nurse. STNA RESPONSIBILITIES AND MAJOR DUTIES: 1. Timely and accurately document all resident related activities in accordance with facility policy. 2. Ensure timely notification of resident related information, including changes in resident condition (example: weight loss, skin condition changes), to a licensed nurse for further direction. 3. Successfully assist all responsible residents with Activities of Daily Living (ADL’s), including but not limited to feeding, bathing, dressing, transferring, ambulation, loco motion, personal hygiene, toileting. 4. Obtain labs/vitals as directed by licensed nurse 5. Observe infection control and safety procedures. 6. Attend and participate in all scheduled staff meetings, educational meetings/in-services and all other meetings related to employment. 7. Assist with and participate in all resident activities to facilitate a home like environment for the residents within assigned facility. 8. Ensure compliance with all HIPAA regulations. 9. Respectfully engage and welcome all visitors, guests and family members within the facility. 10. Ensure a safe and sanitary living environment for residents. 11. Adhere to all infection control policies within the assigned facility. 12. Ensure the Resident Assessment Instrument (RAI) process is followed in accordance to State & Federal Guidelines. 13. STNA QUALIFICATIONS: EDUCATION AND EXPERIENCE Ability to obtain and maintain State Tested Nursing Assistant (STNA) Certification required. High School Diploma/GED required. Ability to read/write/comprehend the English language in written & verbal form required. Ability to communicate both verbally and non-verbally required. Successfully complete &/or comply with all required background checks to include but not limited to: Ohio BCI&I fingerprint analysis, FBI (if applicable) fingerprint analysis, Nurse Aide Registry, HHS/OIG list of Excluded Individuals/Entities (LEIE) & Ohio Medicaid Provider Exclusion and Suspension List. Valid STNA certification to work within a long term care facility within the State of Ohio. Dependable transportation required. CPR certification preferred.
Position Summary: The Assistant Director of Nursing (ADON) assists in the planning, organization, development, and supervision of nursing services in accordance with federal, state, and local regulations, as well as facility policies. The ADON plays a key leadership role in ensuring quality resident care, supporting clinical staff, and maintaining regulatory compliance in collaboration with the Director of Nursing. Key Responsibilities: Leadership & Staff Supervision: Support the DON in managing day-to-day clinical operations. Supervise nursing staff (RNs, LPNs, CNAs), providing leadership and guidance. Assist with hiring, orientation, training, and evaluation of nursing personnel. Monitor staff performance and provide coaching, support, and discipline as needed. Assist with staffing schedules and ensure adequate coverage on all shifts. Resident Care & Clinical Oversight: Monitor resident care and ensure care plans are followed and updated as needed. Conduct rounds to assess resident condition and quality of care. Respond to changes in resident status and ensure timely physician and family communication. Participate in admission, transfer, and discharge planning. Oversee medication administration and clinical procedures for accuracy and safety. Regulatory Compliance & Quality Assurance: Assist in preparing for surveys, inspections, and audits. Help lead the Quality Assurance and Performance Improvement (QAPI) program. Ensure documentation and charting are complete and accurate. Enforce compliance with infection control, safety, and resident rights standards. Communication & Coordination: Serve as a liaison between nursing staff and administration. Communicate effectively with residents, families, and interdisciplinary teams. Attend and participate in care plan meetings, department head meetings, and in-services. Qualifications: Current RN license in the state of employment, in good standing Bachelor of Science in Nursing (BSN) preferred; ADN with relevant experience acceptable Minimum of 2–3 years of nursing experience in long-term care, with leadership or supervisory experience preferred Must be IP certified Strong understanding of federal and state nursing home regulations (CMS, OBRA, etc.) Excellent leadership, communication, and interpersonal skills Proficient in electronic medical records (EMR) and documentation standards Current CPR/BLS certification Physical Requirements: Ability to lift/move up to 50 lbs Frequent walking, standing, and physical activity throughout the facility Ability to respond to emergencies at any time Work Schedule: Typically Monday–Friday, with flexibility for weekends, holidays, and on-call coverage as needed
The Director of Nursing (DON) is responsible for the overall management of the nursing department and the coordination of resident care services in a long-term care facility. The DON ensures that care is delivered in accordance with state and federal regulations, facility policies, and professional nursing standards. This leadership role involves staff supervision, clinical oversight, regulatory compliance, and strategic planning. Key Responsibilities: Leadership & Management: Lead and manage all nursing staff including RNs, LPNs, CNAs, and Med Techs. Hire, train, supervise, and evaluate nursing personnel. Establish and maintain staffing schedules to ensure appropriate coverage. Foster a positive and collaborative work environment with open communication. Participate in facility leadership meetings and decision-making processes. Clinical Oversight: Oversee development, implementation, and evaluation of individualized care plans. Ensure all resident care is delivered according to clinical standards and care plans. Monitor residents’ health status and outcomes; lead interventions for changes in condition. Collaborate with medical providers and interdisciplinary teams to coordinate care. Ensure accurate and timely documentation and medical record management. Regulatory Compliance & Quality Assurance: Ensure compliance with all state, federal, and local regulations (e.g., CMS, OSHA, HIPAA). Prepare for and participate in state surveys, audits, and inspections. Lead Quality Assurance and Performance Improvement (QAPI) efforts. Investigate incidents, accidents, or complaints and implement corrective actions. Maintain up-to-date knowledge of nursing home regulations and best practices. Education & Training: Develop and conduct ongoing staff education, in-services, and competency evaluations. Promote professional growth and licensure compliance among nursing staff.
