Summary Supports the department Manager and Director in the daily operations of the department; and in mentoring and professional development of staff. Responsible and accountable for translating the mission, strategic goals and department priorities. Location: Fort Wayne Medical Oncology and Hematology Pay Range: $24.83 - $46.12 Position Summary Provides safe, accurate and efficient patient care, including but not limited to: patient assessment, review of labs and vital signs, medication safety, dose verification, and drug administration. Paradigm is looking for a 0.75 - 1.0 FTE (30 - 40 hours per week), benefited Field Nurse Case Manager to cover the Fort Wayne, IN, area . This is a salaried position with a base salary range at 0.75 FTE of $65,000 - 69,000. At 1.0 FTE, base salary range is $86,500 - 92,000. A Field Case Manager role is a home-based position with travel, up to 2 hours one-way (4 hours round trip), to doctors’ offices, hospitals, and various other locations. This individual is responsible for the medical case management of work-related injuries, which includes assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. The Case Manager works with insurance carriers, medical care providers, attorneys, employers, and employees, and closely monitors the progress of the injured worker and reports results back to the employer and insurance carriers. Qualifications Current, unencumbered Registered Nurse (RN) license in Indiana Experience in case management of workers comp cases, strongly preferred Recommended certifications (CCM, CRRN, CDMS, CRC) strongly preferred; will need to obtain within two years of hire date Skills required for success: organization and timeline adherence skills, PC and technology skills, communication skills (written, verbal, and interpersonal) Ability to operate autonomously with minimal oversight Skilled at patient education Valid driver’s license and good driving history 
The ED Registered Nurse functions under the supervision and direction of the ED Manager, ED Coordinator, and Physicians in order to assist in providing and assessing direct emergency and outpatient care. The ED RN provides professional nursing care to medically ill and trauma patients of various criticalities. The AMH ED RN uses the Emergency Severity Index (ESI) triage tool to prioritize care for patients presenting to the ED. The ED RN coordinates care planning with other disciplines to ensure that patient needs are met, participates in inpatient and family education, provides an environment conducive to patient safety and comfort, seeks educational resources, and creates a learning experience to enhance and maintain current knowledge and skill for his or her continuing competence in nursing practice and individual professional growth.
