JOB SUMMARY/RESPONSIBILITIES: • Develops and implements a plan of treatment to meet patients’ needs in accordance with the principles and practices of a registered nurse. TYPICAL PHYSICAL DEMANDS: • Sitting, finger dexterity, seeing, hearing, speaking. • Continuous: reaching above, at and below shoulders; repetitive arm/hand motion. • Frequent: standing, walking, stooping/bending, gripping of objects, and twisting body. • Occasional: lifting and carrying usual weight up to 50 pounds, pushing/pulling 50 pounds of force. III. TYPICAL WORKING CONDITIONS: • Subjected to adverse environmental conditions like continuous exposure to blood borne pathogens. IV. MINIMUM QUALIFICATIONS: A. EDUCATION/CERTIFICATION AND LICENSURE: • Current Hawaii State license as a Registered Nurse. • Associate's or bachelor's degree in nursing • Valid U.S driver’s license, vehicle and vehicle insurance as required by Hawaii State Law. • Driver's abstract reflecting evidence of good driving record. • Current BLS certification. B. Assumes primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating the plan of treatment. Assesses patient needs and obtains data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status and incorporate that data into the plan of care. Makes the initial evaluation and re-evaluates the patient’s nursing needs during each visit. Makes follow up visits according to patient, family, and facility needs. Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care. Revises the plan of care in response to identified patient care issues and notifies the Clinical Supervisor, physician, facility, and other team members. Counsels, instructs, and includes the patient, facility, and family in following the Interdisciplinary Plan of Care. Teaches behavioral health philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.). Initiates, supervises, and documents observance of the behavioral health aide and LPN delivery of care per agency policy. Informs supervisor and/or facility supervisor of unusual or potentially problematic nursing facility issues. Provides appropriate support at time of death and perform bereavement assessment. What is Required? Current, unrestricted state RN licensee 1+ years of experience in a clinical care setting as a nurse Able and willing to travel within branch/office coverage area. Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws. Performs in accordance with physician’s orders and under the supervision of the Clinical Manager. What you will do in this role : Assesses home care patients identifying physical, psychosocial and environmental needs as evidenced by documentation, clinical records, case conferences, team reports, call-in logs and on-site evaluations. Completes OASIS, assessment and visit paperwork according to agency policy. Assures clinical notes accurately indicate continuing communication and coordination of services with the physician, other interdisciplinary team members and patient/family/caregiver. Communicates significant findings, problems and changes to Clinical Manager and physician, and documents all findings, communications, and appropriate interventions. Supervises and provides clinical direction to home health aides and LPNs/LVNs to ensure quality and continuity of services provided. Responsible for participating in on-call rotation and emergency call according to agency policy.
