Clinical background of wound care; including but not limited to chronic, debilitating, stalled, or infected wounds Must be comfortable with diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers. Must be comfortable staging pressure injuries according to the NPIAP Must be familiar with wound care best practice and advanced wound care products; collagens, alginates, foams, silvers, etc. Must have experience with wound vacs and other DME, such as pressure relieving/ reducing devices Strong preference to having wound certification or be open to getting certified Experience working in a long-term care setting. Position Summary The Wound Care APP (NP/PA) provides advanced clinical evaluation and management of patients with acute and chronic wounds. This role partners with physicians, nurses, and specialty teams to drive healing outcomes, reduce complications (including pressure injuries), and ensure safe transitions of care. The APP functions as a clinical leader within an academic, consult-driven, and patient-centered environment. Main Tasks • Perform wound assessments, including cause, stage, infection risk, perfusion, pain, and related conditions. • Treat complex wounds such as pressure injuries, diabetic foot ulcers, venous or arterial ulcers, surgical wounds, trauma wounds, and ostomy issues. • Develop and implement evidence-based treatment plans for topical care, compression, offloading, NPWT, and escalation. • Collaborate with teams like Vascular Surgery, Plastics, Podiatry/Ortho, Infectious Disease, Nursing, Nutrition, PT/OT, and Case Management. • Help prevent pressure injuries through rounds, risk checks, education, and quality initiatives. • Document in EMR, support imaging and measurement workflows. • Educate patients and families, plan discharges, and coordinate follow-up care.
