Job Title: LPN Team Case Assistant - Home Care Location: Monroe County Hours Per Week: 20 hours Schedule: Day/Evening, 6 hour shifts, Friday 12pm-5pm, Saturday 8am-5pm, Sunday 9am-4pm SUMMARY: Utilizing clinical experience, coordinates patient services under the direction of the team manager/designee. This includes but not limited to prioritization and coordination of daily patient schedules, requesting additional services such as aide, therapy, social work, interpreter services, and ensuring that referrals for services are appropriately assigned in a timely fashion. RESPONSIBILITIES: Validates supplemental orders and processes per policy. Remains available to assist in other duties deemed appropriate to the scope of practice of an LPN. Manages multiple phone calls in a timely and professional manner providing appropriate follow up to assure callers’ needs are met. Provides field staff assistance and resources for appropriate day to day issues as they arise under the direction of the team manager/designee. REQUIRED QUALIFICATIONS: Graduate of an accredited school of licensed practical nursing. A valid NYS license as a Licensed Practical Nurse. PREFERRED QUALIFICATIONS: One year of experience in home care setting or medical office preferred. Excellent interpersonal skills and customer service skills. Ability to multi-task in a fast paced environment. Proficient use of Microsoft office preferred.
